A. For new building or new use:

3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains less than 2/1 0 of I % lead.
5. Commercial building, industrial building, multiple residences and similar buildings and installations, a certificate
of Code Compliance from architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
B. For existing buildings (prior to April 9, 1957) non-conforming uses, or buildings and "pre-existing" land uses:

On July 12lh, I met with the Williams to review their inspection.
The den has a shed roof that connects to the main gable of the house. At the low end
which is at the front of the house, there is a roof-overhang that has a continuous
aluminum vent centered in the soffit that runs the entire length. Since the addition was
built, a perforated vinyl bead board has been installed for aesthetics. At the top of the
shed roof where it meets the main roof, there are at least three 12" x 12" opening from
the shed roof into the attic of the house. Therefore" the venting at both the top and the
bottom of the shed roof provide the necessary venting of that roof.
I provided a Compliance Certificate with the application. The energy calculations include
the den and indicate there is insulation in the walls, ceiling and floor. I have attached a
copy of that certificate.
John Rose and I checked the basement for access to the crawl space of the den. The
existing house has a poured concrete foundation. The den addition has a concrete block
foundation wall. Cutting an opening from the basement to the crawl space is both
expensive and messy because the basement contains the laundry, mechanical equipment
and storage. It wOqld be extremely difficult to control the dust from the cutting of the
concrete. There is no plumbing or electrical in the crawl space. The electrical was run
through the walls, and the heat is hot water which also runs along the walls. The crawl
space has venting as required by code.
Because R408.3 Access in the code does not specifY the access is to be from the
basement, we would like permission to install the required 18" x 24" access from the
exterior of the den. The concrete block of the foundation can be easily removed. A pit
would be dug on the west side of the den as shown on the attached survey and the soil .
retained with treated 4x4s with gravel in the bottom. The access hole would be made into
the concrete block wall and provided with a removable panel.

INSTRUCTIONS
a. This application MUST be completely filled in by typewriter or in ink and submitted to the Building Inspector with 4
sets of plans, accurate plot plan to scale. Fee according to schedule.
b. Plot plan showing location of lot and of buildings on premises, relationship to adjoining premises or public streets or
areas, and waterways.
c. The work covered by this application may not be commenced before issuance of Building Permit.
d. Upon approval of this application, the Building Inspector will issue a Building Permit to the applicant. Such a permit
shall be kept on the premises available for inspection throughout the work.
e. No building shall b~ occupied or used in whole or in part for any purpose what so ever until the Building Inspector
issues a Certificate of Occupancy.
f. Every building permit shall expire if the work authorized has not commenced within 12 months after the date of
issuance or has not been completed within 18 months from such date. If no zoning amendments or other regulations affecting the
property have been enacted in the interim, the Building bspector may authorize, in writing, the extension ofthe permit for an
addition six months. Thereafter, a new permit shall be reqnired.
