2. Final Approval from Health Dept. of water supply and sewerage-disposal (S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains less than 2/1 0 of I % lead.
5. Commercial building, industrial building, multiple residences and similar buildings and installations, a certificate
of Code Compliance from architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
B. For existing buildings (prior to April 9, 1957) non-conforming IIses, or bllildings and "pre-existing" land uses:
I. Accurate survey of property showing all property lines, streets, building and unusual natural or topographic

Re: Permit No. 32759Z - Johnson Residence
Dear Mr. Gillen:
I inspected the new framing, strapping, plumbing, and insulation that was part of the bathroom
and kitchen renovations at the Johnson residence. Inspection entailed opening of ceiling and wall
coverings in order to visually determine the presence and adequacy of underlying components.
Inspection found the damaged second floor joists to have been properly repaired and reinforced
and all headers and strapping shown in the plans prepared by Architechnologies of Mattituck
dated July 16, 2004 were found to have been installed as specified.
Inspection also found the water supply and waste plumbing to be free of leaks and properly
installed.
Based on these observations, and to the best of my knowledge, the framing and plumbing
components and systems are installed in conformance with the Residential Code of New York
State.

Description:
The premises is a two-story, expanded cape cod style wood frame dwelling.
Purpose of the Inspection:
The client had engaged Home Depot to remodel the kitchen, add a first floor bathroom
and remodel the second floor bathroom. The work was in progress, but had stopped.
Our services were engaged to make an inspection of the work completed and to evaluate
the quality of that work.
Cabinets
At the time of the inspection, the kitchen cabinets
were in their location, but installation was
incomplete Three sections of the base cabinets
appeared to have been sized incorrectly. (See
Photos 1,2,&3.) The client reported that
replacemeut cabinets had been sent, but the
contractor has not come back to install them.

As part of the worl<, a new window was installed in
the kitchen and two windows removed and closed in
the first floor hathroom. There was visible evidence
of serious
flaws in
the
installation
of the
kitchen
window.
The most
serious
condition
involves
the framing. At the side jambs it was
observed that a double plate had been cut
through, and the side of the opening was
blocked to the bottom of the plates and then
from the top of the plates to the header. (photo 4 & 5.) The waIl framing at each side
of the window is now discontinuous and, therefore, unstable.
There were also installation problems at the exterior. (photo 6) The flashing at the head
jamb has been crimped and bent. When flashing
is damaged, it will usually leak. In addition the
sheathing at the sides and top was patched-in
with the joints unprotected with flashing or other
materials. It was further observed that the
patched-in sheathing did not match to the
existing, making it impossible to install the
siding

Framing
There were framing problems reported to have
been caused by the plumber. Our observation
of this condition was limited to photographs
laken during the construction. This area was
closed with sheetrock at the time of the
inspection. (photos 5, 11 & 12.)
Photo 12 was reported to be the condition that
existed after the plwnbing bad been installed
and the contractor was beginning to install the
sheetrock. One of the floor joists has been
completely severed to run the waste line.
Another joist has had so much cut away that it
is no longer adequate. This cut-out appears to
have been a mistake as there is no plumbing

serious structural deficiencies.

Where the pipe passes through the joist, there
had been an existing notch which appeared to
have been wm a missing knot. The presence
of this notch should have precluded any
further reduction of the beam, i.e.,cut outs.
By making the cut out for the pipe, a joist that
had been adequate was completely severed.
Had the ceiling been sheetrocked without
repairing the damage, severe settling of the
floor would occur causing damage to the tile
work and possibly the plumbing of the bathroom above. Because of the redundancies
inherent in a frame dwelling, the loads carried by these joists would be transferred
through the subfloor to other joists. This settling, therefore, may not have manifested
itself until well after the contractor had finished the job and received payment. This
would have held up until the subfloor gradually deflected because of the increased span.
As deflection occurred, nails and fasteners would loosen under the additional loads
causing further deflection. In a year or two the deflection would become quite
pronounced and noticeable. As the deflection progressed, further damage would occur
to the finished walls, doors, tile work, etc. Fortunately, this problem was caught and
appears to have been corrected adequately with sister beams.
The final repair to the joists, i.e., the holes cut in the system joists for the pipes was not
observable. The following parameters should have been met.
I. Three must be atleast 1-112" of material above and below any opening.
2. For a round "bored" hole the location must not exceed 3'-10" wm the
support wall.
3. For a square cut hole, the location must not exceed 2'-3" wm the
support wall.
If there is reasonable suspicion that these parameters have not been met, the ceiling
should be opened for inspection.
Recommendations:
It will be necessary, of course, to install the correct cabinets and return the incorrect
cabinets. Since Home Depot reportedly did the measuring and selection, it would be
reasonable that they would take responsibility for any additional labor changes and for
the return of the incorrect cabinets.

The kitchen window must be removed and the opening re-framed properly. The
sheathing must be repaired at all windows and flashed or otherwise protected against
leakage. The siding must then be matched to complete the work.
