WITNESSETH, that the party of the first part, without consideration, does hereby grant and release unto the party
of the second part, the heirs or successors and assigns of the party of the second part forever,
All that certain plot, piece or parcel of land, with the buildings and improvements thereon erected, situate, lying
and being at Fishers Island. Tow~ of Bouthoid. County of Suffolk and State of New York, being more parficelady
bounded and described as set forth on Schedule A hereto.
BEING AND INTENDED TO BE the same promises conveyed to the party of the first part by deed, dated August
11. 1980 and recorded In the Office of the Suffolk County Clerk on September 3, 1980 Uher 8878 page 185.
SUBJECT fights of wa]/described on Schedule A hereto and reverter described below.
This conveyance is made by the party of the first part to the party of the second part on the condition that the
subject premises are utilized by the party of the second part, Its successors and assigns, solely for not-for-prof'~
medical or other not. for. pmffi charitable purposes, including purposes reasonably misted to and in furtherance
of the use of the premises adjoining the subject premises on the south and east (SCTM No. $000-006.00-06.00-
020.007) for a physician's resideoce and home office and/or medical clinic. In the event the subject premises are
ever used for any other purpose (except to the extent the same remain vacant and unoccupie.cl. ), then'and In that
event the subject premises shell revert to the party of the second part or her helms and assigns.
TOGETHER with all dght, title and Interest, if any, of the party of the first part in and to any s{reets and muds
abutting the above described premises to the center lines thereof; TOGETHER with the appurtenances and all
the estate and rights of the party of the §irst part in and to said premises; TO HAVE AND TO HOLD the premises
herein granted unto the party of the second part, the heirs or successors and assigns of the party of the second
part forever.
AND the party of the first part covenants that the party of the first part has not done or suffered anything whereby
the said premises have been encumbered In any way whatever, except as aforesaid.
AND the party of the flint part, In compiianca with Section 13 of the Uen Law, covenants that the party of the first
part will receive the consideration for this conveyance and will hold the right to receive sucfl consideration as a
trust fund to be applied first for the purpose of paying the cost of the Improvement and will apply the same first
to the payment of the cost of the Improvement before using any part of the total of the same for any other
pumese. The word "party" shall be construed as if it read "parties" whenever the sense of this indenture so
requires.

erected, situate, lying and being at Fishers Island, Town of Southold, County of Suffolk
and State of New York, more specifically bounded and described as follows:
BEGINNING at a monument at the intersection of the easterly line of Mansion House Ddve
and the northeasterly line of Central Avenue, said monument being 3679.36 feet North of
a point which is 1906.68 feet West of another monument marking the United Slates Coast
and Geodetic Survey Triangulation Station 'PROS"; and
THENCE RUNNING along said Central Avenue line South 71 degrees 27 minutes 10
seconds East, 100.00 feet to an iron;
THENCE North 11 degrees 46 minutes 00 seconds East, 129.86 feet;
THENCE North 78 degrees 14 minutes 00 seconds West, 7.22 feet;
THENCE North 10 degrees 49 minutes 00 seconds East, 100.00 feet to an iron pipe;
THENCE North 82 degrees 40 minutes 00 seconds East, 180.00 feet to a monument;
THENCE North 18 degrees 05 minules 40 seconds West, 141.80 feet to a monument;
THENCE South 71 degrees 14 minutes 30 seconds West, 263.00 feet to the easterly line
of said Mansion House Drive;
THENCE along the said Mansion House Drive line South 20 degrees 15 minutes 30
seconds East, 33.60 feet to a monument;
THENCE along the said Mansion House Drive line South 07 degrees 15 minutes 30
seconds West, 238.46 feet lo the point of BEGINNING.

SUBJECT TO a right of way ten (10) feet in width for a pole line, the center line of said
right of way being described as follows:
BEGINNING at a point on the westerly line of the above described tract, said point being
3875.35 feet North of a point which is 1881.71 feet West of said "PROS' monument, and
thence running South 86 degrees 20 minutes 00 seconds East, about 107.00 feet to an
easterly line of the above described premises.
SUBJECT TO a right of way ten (10) feet in width for a water line, the center line of said
right of way being described as follows:
BEGINNING at a point on the westerly line of the above described tract, said point being

eastedy line of the above described premises.
