APPLICATION FOR CERTIFICATE OF OCCUPANCY
This application must be filled in by typewriter or ink and submitted to the Building Department with the following:
A. For new building or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or
topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal (8-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains less than 2/10 of 1 % lead.
5. Commercial building, industrial building, multiple residences and similar buildings and installations, a certificate
of Code Compliance from architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
B. For existing buildings (prior to April 9, 1957) non-conforming uses, or buildings and "pre-existing" land uses:
I. Accurate survey of property showing all property lines, streets, building and unusual natural or topographic
features.
2. A properly completed application and consent to inspect signed by the applicant. If a Certificate of Occupancy is
denied, the Building Inspector shall state the reasons therefor in writing to the applicant.

Hi Mike,
This email is to back up what I informed you of this morning.
The following has occurred on the matter of holding the certificate of occupancy for
the above referenced property because of the electrical vault/box encroachment on
the drainage basin property known as SCTM 1000-1-1.10. The Planning Board
held a work session last night and discussed the site plan satisfaction letter dated
May 7, 2007. At this time the Planning Board has no other issue with the electrical
vault placement and still agrees with the conditions of the original satisfaction letter.
The planning Board recommends the clo be issued on the third building as
indicated in the letter.
Peter Harris the Superintendent of Highways has verbally stated that this electrical
vault is ok as long as the clr easement is reviewed to protect the Town and filed to
the satisfaction of the Assistant Town Attorneys legal approval. Additionally, Pete
requested that the location of future electrical vaults within a drainage area be
avoided.

Dear Mr. Kosmynka:
The Suffolk County Water Authority has received your application for water service for the above
referenced parcel. The parcel is designated in the SCW A's Five Year Water Supply Plan for the North
Fork as eligible to receive public water service. Accordingly, this Letter of Water Availability is issued.
An agreement has been signed and executed to install water main on Commerce Drive, Cutchogue, which
will serve your parcel.
This Letter of Water Availability is not to be considered an action by SCWA as defined by the New York
Environmental Quality Review Act or a determination that the parcel is approved for building. You are
responsible for obtaining any other permits or approvals that are required for the proposed project. SCW A
shall not be required to provide water for the parcel until all of the other approvals have been obtained.
SCW A will provide water service to the parcel upon payment of the applicable fees. The fees include a
tapping fee, a key money charge and if applicable, a water main surcharge. Tapping fees start at $950.00
for a 1" service. Key money fees start at $3,000.00 for a 1" service. Surcharge amounts vary depending
on the location of the proposed service. The tapping fees and the surcharge may be financed for existing
residential customers.
