APPLICATION FOR CERTIFICATE OF OCCUPANCY
This application must be filled in by typewriter or ink and submitted to the Building Department with the following:
A. For new building or new use:
I. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or
topographic features,
2. Final Approval from Health Dept. of water supply and sewerage-disposal (S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains less than 2/1 0 of 1 % lead.
5. Commercial building, industrial building, multiple residences and similar buildings and installations, a certificate
of Code Compliance from architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
B. For existing buildings (prior to April 9, 1957) non-conforming uses, or buildings and "pre-existing" land uses:
. 1. Accurate survey of property showing all property lines, streets, building and unusual natural or topographic
features.
2. A properly completed application and consent to inspect signed by the applicant. If a Certificate of Occupancy is
denied, the Building Inspector shall state the reasons therefor in writing to the applicant.

To Whom This May Concern:
Please take notice that we are returning your building permit application for construction
of additions and alterations at 3130 Rocky Point Road, East Marion, SCTM# 21-4-12, is
being returned as incomplete. It has come to our attention that, as of January I, 2003,
such applications must meet the new New York State building code. As a result, The
Town of South old has specific requirements that must be on every set of plans submitted
to this office. A number of those requirements are not listed on your plans and we are
unable to determine whether or not your plans meet the requirements for our area.
Weare sorry for an inconvenience this may cause. Please feel free to re-submit the
application when you have address the above referenced issues. I have enclosed a
checklist of specific items necessary to move forward with this permit. If you have any
questions, please feel free to contact this office at (631) 765-1802 between the hours of
8:00 a.m. and 4:00 p.m.

b. Plot plan showing location of lot and of buildings on premises, relationship to adjoining premises or public streets or
areas, and waterways.
c. The work covered by this application may not be commenced before issuance of Building Permit.
d. Upon approval of this application, the Building Inspector will issue a Building Permit to the applicant. Such a permit
shall be kept on the premises available for inspection throughout the work.
e. No building shall be occupied or used in whole or in part for any purpose what so ever until the Building Inspector
issues a Certificate of Occupancy.
f. Every building permit shall expire if the work authorized has not Commenced within 12 months after the date of
issuance or has not been completed within 18 months from such date. If no zoning amendments or other regulations affecting the
property have been enacted in the interim, the Building Inspector may authorize, in writing, the extension of the permit for an
addition six months. Thereafter, a new permit shall be required.

As l'er our phone oonversation last week, I bave prollided the following clarifioations and
changes.
1. l htive added brtll:flrs to the fence top - oee the attached sketch and detail. I have
doubled the lop plale. Thl. effectively reduces the: spall to S'-O" simpl)' supported, with.
4 '-0" cl!ll1tilevcr. The continuous aclioll of the beam should be considered here, and that
the fence is approximately 50% open.
2. The second floor deck has always been braced diagonally and anchored to thl!
house. With the additional bracing provided at the fence, no "kicleer" should be required.
3. I have !l.\'Ovided a post bllse that securely anchors the post w the ground
4. r Iulve prnvided the cuts for th.. various bracina and ties that I am u.ing that at"
referenced. on the original drawings. The cuts provide the nail schedules ftJ1d load
c"paciljes~

a.o This application MUST be completely filled in by typewriter or in ink and submitted to the Building Inspector with 3
sets of plans, accurate plot plan to scale. Fee according to schedule.
b. Plot plan showing location of lot and of buildings on premises, relationship to adjoining premises or public streets or
areas, and waterways.
c. The work covered by this application may not be commenced before issuance of Building Permit.
d. Upon approval of this application, the Building Inspector will issue a Building Permit to the applicant. Such a permit
shall be kept on the premises available for inspection throughout the work.
e. No building shall be occupied or used in whole or in part for any purpose what so ever until the Building Inspector
issues a Certificate of Occupancy.
f. Every building permit shall expire if the work authorized has not commenced within 12 months after the date of
issuance or has not been completed within 18 months from such date. If no zoning amendments or other regulations affecting the
property have been enacted in the interim, the Building Inspector may authorize, in writing, the extension of the permit for an
addition six months. Thereafter, a new permit shall be required.
APPLICATION IS HEREBY MADE to the Building Department for the issuance of a Building Permit pursuant to the
Building Zone Ordinance of the Town of South old, Suffolk County, New York, and other applicable Laws, Ordinances or
Regulations, for the construction of buildings, additions, or alterations or for removal or demolition as herein described. The
applicant agrees to comply with all applicable laws, ordinances, building code, using code, and regulations, and to admit

GENERAL NOTES
1. Scope of Work:
New exterior deck and balcony.
No change of use, egress or occupancy.
2. Contracto(s General Responsibilities: All work indicated in the
Construction Documents is to be done under this contract by the General
Contractor or his sub-contractors, unless specifically noted otherwise.
During the bid process, if the Contractor finds any conflicts, ambiguities or
omissions in any of the Construction Documents, or is in doubt as to their
meaning, he must bring the issue to the attention of the Architect before
the submission of the Bid. The general Contractor shall be responsible for
coordinating the work of other trades.
3. The contractor shall coordinate all work procedures with the stipulations
of local authorities, building management or board of directors.
4. The contractor shaillay out his own work, and shall provide all
dimensions required for other trades: plumbing, electrical, cabinetry, etc.
5. Plumbing and electrical work shall be performed by persons licensed in
their trades, who shall arrange for and obtain inspections and required sign
oils.
6. The contractor shall do all cutting, patching, and repairing as required to
perform all of the work indicated on the drawings, and all other work that
may be required to complete the job.
7. All piping and wiring shall be removed to a point of concealment and
shall be properiy capped or plugged.
8. Contractor is to meet all Insurance and Filing Requirements Before
commencing work, the Contractor shall file all required Certificates of
insurance with the Suffolk County Department of Buildings and shall pay
all necessary costs and fees associated with obtaining a work permit as
required by governing agencies. The work permit as applicable shall be
posted in a conspicuous place within the affected area. The owner shall be
responsible only for the initial pre-filing with the Building Depariment.
9. Conformance with New York State Building Code and Related
Agencies. All work shall conform with the requirements of the New York
Stale Building Code, Fire Department regulations, Utility Company
Regulations and all other agencies having jurisdiction over this project,
including management company regulations.
10. Protection of Existing Construction. The Contractor shall be
responsible for the protection of all existing construction to remain and any
new items installed under separate contracts including carpeting and
T enanfs equipment through substantial completion of the project. The
Contractor shall bear sole responsibility for any damage andlor injuries
related to the execution of the construction work and as a result of
inadequate protection measures.
11. Use of drawings: Drawings are not to be scated: use dimensions on
drawing only. Large scale details govern over small scale drawings or
details. Dimensions shown on the Drawings are from finished face to
finished face, or fromno existing face, or as noted. Conflicts between
Architectural and Engineering drawings shall be brought to the attention of
the Architect for determination concerning issue in conflict.

in the Contract Documents, but necessary for the completion of any part of
the work shall be included at no additional cost to the Tenant.
13. Field Verification of Site conditions: The Contractor shall visit the site
and verify all conditions shown on the drawings. Any discrepancies
between the drawings and existing conditions shall be reported to the
Architect prior to submission of the bid.
14. Public Safety: The Contractor is responsibie to maintain the safety of
the public and adjoining property during the construction operation.
15. Temporary Services: Contractor shall be responsible for fumishing,
installing, maintaining, moving and removing all temporary services
necessary and required to complete the work of this contract. Temporary
services shall meet all applicable requirements of local codes, laws
ordinances and Regulations. The contractor may avail himseWof the
existing services and new permanent services within the building and
project site with the approval of the building management however the use
of such services shall not in any way relieve the contractor of his
responsibility for providing the required temporary services.
16. Firestopping and Blocking: Contractor shall install all firestops or
firestopping material as required by New York State Building Code and Fire
Code regulations.
17. Openings In Fire-Rated-Walls: All openings in rated walls and
enciosures shall be protected with fire rated protective devices as per the
New York State Building Code and Fire Code.
18. Sheetrock Termination Accessories: All outside comers at drywall
partitions shall have metal corner bead; taped and spackled. All sheetrock
shall extend a minimum of 6' above line of acoustical tile unless noted
otherwise. All sheetrock at window mullions and other similar conditions
shall receive 'j' bead.
1 g. General and Final Clean-Up. All general clean-up work is the
responsibility of the Contractor. Premises must receive a finai cleaning
prior to turning the premises over to the tenant, and prior to substantial
Completion. Final cleaning shail include repairs to all damaged surfaces
and equipment that are a result of this work. All Windows, door frames,
metal work, mirrors and both sides of glass surfaces shall be cleaned at
finai clean-up.
20. Schedule. Contractor shall endeavor to be substantially complete with
his portion of the work in a period of eight weeks from start of construction.
Start of construction is defined as date initiai deposit is received for work or
date building department folder is received for permitting. -Contractor shall
submit a schedule of construction activities within 5 days after award of the
project.
21. Warrantee. all work shall be warranted against defects for a period of
one year from date of Final Payment.
22. The architect has not been retained to provide supervision.

GENERAL NOTES
1. Scope of Worl<:
New exterior deck and balcony.
No change of use, egress or occupancy.
2. Contracto(s General Responsibilities: All worl< indicated in the
Construction Documents is to be done under this contract by the General
Contractor or his sub-contractors, unless specifically noted otherwise.
During the bid process, if the Contractor finds any conflicts, ambiguities or
omissions in any of the Construction Documents, or is in doubt as to their
meaning, he must bring the issue to the atlention of the Architect before
the submission of the Bid. The general Contractor shall be responsible for
coordinating the work of other trades.
3. The contractor shail coordinate ail work procedures wi1h the stipulations
of local authorities, building management Of board of directors.
4. The contractor shaillay out his own worl<, and shail provide all
dimensions required for other trades: plumbing, electrical, cabinetry, etc.
5. Plumbing and electrical worl< shall be perfonned by persons licensed in
their trades, who shail arrange for and obtain inspections and required sign
ofts.
6. The contractor shail do all cutting, patching, and repairing as required to
perform ail of the work indicated on the drawings, and all other worI< that
may be required to complete the job.
7. Ail piping and wiring shail be removed to a point of concealment and
shail be prope~y capped or plugged.
8. Contractor is to meet ail Insurance and Filing Requiremenls Before
commencing worl<, the Contractor shall flle all required Certificates of
Insurance with the Suffolk County DepMffient of Buildings and shall pay
ail necessary costs and fees associated with obtaining a worI< pennit as
required by goveming agencies. The worl< pennit as applicabie shall be
posted in a conspicuous place within the affected area. The owner shall be
responsible only for the initial pre-filing wi1h the Building Depa~nt.
g. Confonnance with New Yorl< State Building Code and Related
Agencies. Ail worl< shail confonn with the requiremenls of the New Yorl<
State Building Code, Fire Department regulations, Utililly Company
Regulations and ail other agencies having jurisdiction over this project,
including management company regulations.
10. Protection of Existing Construction. The Contractor shall be
responsible for the protection of ail existing construction to remain and any
new i1ems instailed under separate contracts including ClI]l8ting and
Tenant's equipment through substantial completion of the project. The
Contractor shail bear sole responsibility for any damage and/or injuries
related to the execution of the construction worl< and as a resu~ of
inadequate protection measures.
11. Use of drawings: Drawings are not to be scaled: use dimensions on
drawing only. Large scale details govem over small scale drawings or
details. Dimensions shown on the Drawings are from finished face to
finished face, or fromno existing face, or as noted. Conflicts between
Architectural and Engineering drawings shail be brought to the attention of
the ArcMect for detennination conceming issue in conflict.
12. Compieted Project: Minor details not compietely indicated or specified
in the Contract Documents, but necessary for the completion of any p~ of
the work shail be included at no additional cost to the Tenant.
13. Field Verification of Site conditions: The Contractor shall visit the site
and verify ail conditions shown on the drawings. Any discrepancies
between the drawings and existing condttions shail be reported to the
Architecl prior to submission of the bid.
14. Public Safety: The Contractor is responsible to maintain the safety of
the public and adjoining property during the construction operation.
15. Temporary Services: Contractor shall be responsible for fumishing,
installing, maintaining, moving and removing all temporary services
necessary and required to complete the worl< of this contract. Temporary
services shall meet ail applicable requirements of local codes, laws
ordinances and Regulations. The contrector may avail himse~ of the
existing services and new pennanent services within the building and
project site with the approval of the building management however the use
of such services shail not in any way relieve the contractor of his
responsibility for providing the required temporary services.
16. Firestopping and Blocking: Contractor shall install all firestops or
firestopping material as required by New Yorl< State Building Code and Fire
Code regulations.
17. Openings In Fire-Rated-Wails: Ail openings in rated walls and
enclosures shail be protected with fire rated protective devices as per the
New Yorl< State Building Code and Fire Code.
18. Sheetrock Tennination Accessories: Ail outside comers at drywail
partitions shail have metal comer bead; taped and spackled. All sheetrock
shail extend a minimum of 6" above line of acoustical tile unless noted
otherwise. Ail sheetrock at window mullions and other simiiar conditions
shail receive oj" bead.
19. General and Final Clean-Up. Ail general clean-up worl< is the
responsibility of the Contractor. Premises must receive a final cleaning
prior to tuming the premises over to the tenant. and prior to substantial
Completion. Final cleaning shail include repairs to all damaged surfaces
and equipment that are a result of this work. Ail Windows, door frames,
metal worl<, mirrors and both sides of glass surfaces shail be cleaned at
finai clean-up.
20. Schedule. Contractor shall endeavor to be substantially compiete with
his portion of the worl< in a period of eight weeks from start of construction.
Start of construction is defined as date initial deposit is received for worI< or
date building department folder is received for pennitting. -Contractor shall
submit a schedule of construction activities within 5 days aIler award of the
project.
21. Warrantee. all worl< shall be warranted against defects for a period of
one year from date of Final Payment.
22. The architect has not been retained to provide supervision.
