This application must be filled in by typewriter or ink and submitted to the Building Department with the following:
A. For new building or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or
topographic features.
2. Final Approval from Health Dept. of water supply and sewerage -disposal (S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains less than 2/10 of I % lead.
5. Commercial building, industrial building, multiple residences and similar buildings and installations, a certificate
of Code Compliance from architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
B. For existing buildings (prior to April 9, 1957) non -conforming uses, or buildings and "pre-existing" land uses:
1. Accurate survey of property showing all property lines, streets, building and unusual natural or topographic
features.
2. A properly completed application and consent to inspect signed by the applicant. If a Certificate of Occupancy is
denied, the Building Inspector shall state the reasons therefor in writing to the applicant.

PERMIT
The owner of the above referenced project was granted a building permit back in 1988 to convert an attached
garage into habitable space. The permit included a brick chimney for a fireplace as well as a new wood deck in
the back of the house.
The contractor involved in this project failed to contact the Building Department for any of the required
inspections. As a result the legal occupation of the constructed premises is still pending.
The most recent Building Permit Application dated October 24, 2005 relates to a proposed addition of a family
room in the back of the house, which will eliminate the wood deck.
I inspected the work that was done in regards to the conversion in 1988 and arrived at the conclusion that the
construction was executed in compliance with the NYS Building Code in effect at that time.
The garage Floor was raised to create a crawl space with two access openings. The floor system is 2x10's @ 16"
O.C. which is in compliance with the required 40 psf live load for residential living areas. Floor, walls and ceiling
are insulated. Light and Air requirements for the proposed living room extension are sufficient.
The exterior brick chimney is supported on a foundation of solid masonry and extends beyond the ridge. The
masonry fireplace has a raised hearth. Firebox and hearth were built as per code.
