This application must be filled in by typewriter or ink and submitted to the Building Department with the following:
A. For new building or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or
topographic features.
2. Final Approval from Health Dept. of water supply and sewerage -disposal (S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
� 4. Sworn statement from plumber certifying that the solder used in system contains less than 2/10 of 1% lead.
i 5. Commercial building, industrial building, multiple residences and similar buildings and installations, a certificate
of Code Compliance from architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
B. For existing buildings (prior to April 9,1957) non -conforming uses, or buildings and "pre-existing" land uses:
1. Accurate survey of property showing all property lines, streets, building and unusual natural or topographic
features.
2. A properly completed application and consent to inspect signed by the applicant. If a Certificate of Occupancy is
denied, the Building Inspector shall state the reasons therefor in writing to the applicant.

1. a. This application MUST be completely filled in by typewriter or in ink and submitted to the Building Inspector with 4
sets of plans, accurate plot plan to soale. Fee according to schedule.
b. Plot plan showing location of lot and of buildings on premises, relationship to adjoining premises or public streets or
areas, and waterways.
c. The work covered by this application may not be commenced before issuance'oi$uilding Permit.
d. Upon approval of this application, the Building Inspector will issue a Building Permit to the applicant. Such a permit
shall be kept on the premises available for inspection throughout the work.
e. No building shall be occupied or used in whole or in part for any purpose what so ever until the Building Inspector
issues a Certificate of Occupancy.
f. Every building permit shall expire if the work authorized has not commenced within 12 months after the date of
issuance or has not been completed within 18 months from such date. If no zoning amendments or other regulations affecting the
property have been enacted in the interim, the Building Inspector may authorize, in writing, the extension of the permit for an
addition six months. Thereafter, anew pgrnfit ghall be required. .. '

R602.8 Fireblocking required. Fireblocking shall bi; provided to cut off all concealed draft openings
(both vertical and horizontal) and to form an effective fire barrier between stories, and
between a top story and the roof space. Fireblocking shall be provided in wood -frame
construction in the following locations.
1. In concealed spaces of stud walls and partitions, including furred spaces, at the ceiling and
floor levels. Concealed horizontal furred spaces shall also be fireblocked at intervals not
exceeding 10 feet (3043 mm). Batts or blankets of mineral or glass fiber shall be allowed as
fireblocking walls constructed using parallel ,rows of studs or staggered studs.
2. At all interconnections between concealed vertical and horizontal spates such as occur at
soffits, drop ceilings and cove ceilings.
3. In concealed spaces between stair stringers w tie top and bottom of the run. Enclosed
spaces under stairs shall comply with Section 1314.8.
4. At openings around vents, pipes and ducts at ceiling and floor level, to resist the free
passage of flame and products of combustion.
5. For the fireblocking of chimneys and fireplaces, see Section 81001.16.
6. Fireblocking of cornices of a two-family dwelling is required at the line of dwelling unit
separation.
R602.8.1 Materials. Except as provided in Section 8602 8, Item 4, fireblocking shall consist of
2 -inch (51 mm) nominal lumber, or two thicknesses ofd -inch (25.4 mm) nominal lumber
with broken lap joints, or one thickness Of 23/32 -inch (19.8 rim) wood structural panels
with joints (backed by 23/32 -inch (19.8 nun) wood structural panels or one thickness of
3/4 -inch (19.1 mm) particleboard with johts backed by 3/4 -inch (19.1 mm) particleboard,
1/2 -inch (121.7 mm) gypsum board, or 1kG-incl, (6.4 mm) cement -based millboard. Batts or
blankets of mineral wool or glass fiber installed in such a manner as to be securely
retained in place shall be permitted as an acceptable fire block. Loose -fill insulation
material shall not be used as a fire block , mless specifically tested in the form and mariner
intended for use to demonstrate its ability to refrain in place and to retard the spread of
fire and hot gases.
R602.8.1.1 Unli:ced fiberglass. Unlaced fiberglass I. iii insulation used as fireblocking shall fill tie
entire cross section of the wall cavity to a minimum height of 16 inches (406 nun)
measured vertically. When piping, con::iuit Or similar obstructions are encountced,
