This application must be filled in by typewriter or ink and submitted to the Building Department with the following:
A. For new building or new use:

2. Final Approval from Health Dept. of water supply and sewerage -disposal (S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains less than 2/10 of 1 % lead.
5. Commercial building, industrial building, multiple residences and similar buildings and installations, a certificate
of Code Compliance from architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
B. For existing buildings (prior to April 9, 1957) non -conforming uses, or buildings and "pre-existing" land uses:
1. Accurate survey of property showing all property lines, streets, building and unusual natural or topographic
features.
2. A properly completed application and consent to inspect signed by the applicant. If a Certificate of Occupancy is

I inspected the strapping at the Reeve residence on Hilltop Path in Southold New York and found
the strapping to be installed in accordance with the construction plans and the Residential Code
of the State of New York.
I also inspected the building foundation walls and found them to be constructed with the rebar as
shown in the drawings. The rebar in the walls is not structurally required for a building of this size
and thus exceeds standards specified in the Residential Code of the State of New York.
I also inspected the rigid foam insulation installed on the roof and found it to be in accordance
with the plans and the Residential Code of the State of New York.

Dear Mr. Reeve:
The Department of Environmental Conservation has completed a revicmc of N our recent jurisdictional
inquiry under the Tidal Wetlands Act for a proposal to winterize and renovate an existing single farm]%
dwelling on the referenced property as shown on the enclosed survey by Anthony Lewandowski L.S., dated
September 18, 2000, last revised November 1. 2000.
Based on the information you have submitted, we have determined that the parcel is located land card of the
ten -foot above mean sea level elevation contour as shom.n on the referenced sur\c\ and is therefore outside
the area regulated b} this Department under the Tidal Wetlands Act and the Tidal Wetlands Land Use
Regulations (6 NYCRR Part 661).
Therefore. in accordance with Part 661, no permit is required under the Tidal Wetlands Act for the project
depicted on the referenced plan. Please be advised, however, that no construction. sedimentation or
disturbance of an kind mai take place seaward of the tidal actlands jurisdictional boundary (the ten -foot
elevation contour) without a permit. It is \our responsibilit} to ensure that all neccssan precautions are
taken to prevent am sedimentation or other alteration or disturbance to the ground surface or ccgctation
m\ithin Tidal Wetlands jurisdiction which mac result from \ our project. Such precautions may include
maintaining an adequate work arca between the tidal mcetland jurisdictional boundary and \our project (I.e.
a 1S to 20' mcide constriction arca) or erecting a temporary fence, barrier or ha\ bale berm.
