This application must be filled in by typewriter or ink and submitted to the Building Department with the following:
A. For new building or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or
topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal (S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains less than 2/1 0 of 1 % lea~ ')
5. Commercial building, industrial building, multiple residences and similar buildings and installation_~rtlficate
of Code Compliance from architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
B. For existing buildings (prior to April 9, 1957) non-conforming uses, or buildings and "pre-existing" I,md uses:
I. Accurate survey of property showing all property lines, streets, building and unusual natural oitopographic
features.
2. A properly completed application and consent to inspect signed by the applicant. If a Certificate of Occupancy is
denied, the Building Inspector shall state the reasons therefor in writing to the applicant.

The following action was taken at the Southold Town Board of
Trustees meeting on February 25, 1998:
RESOLVED, that the Southold Town Board of Trustees approve the
request for a Waiver to repair and replace roof and windows on
an existing garage. Gutters and leaders will be added to
contain runoff, and to replace wood cap on top of concrete
retaining wall. Located: Sailor's Needle Road, Mattituck.
However, this does not constitute approval from any other
agencies.

A review has been made of your proposal to replace 2x8 wood cap on existing concrete
wall and roof on existing garage.
Based on the information you have submitted, the New York State Department of
Environmental Conservation has determined that the project is listed in Part 661.5 of
6NYGRR (Tidal Wetlands Land Use Regulations) as a use not requiring a permit or
notification letter of approval. Therefore, no permit is required under the Tidal
Wetlands Act (Article 25 of the Environmental Conservation Law).
Please note that any additional work, or modification to the project as described, may
require authorization by this Department. Please contact this office if such are
contemplated.
Please be further advised that this letter does not relieve you of the responsibility of
obtaining any necessary permits or approvals from other agencies.
