DATE INSPECTOR ENT Corp 218 Main Street# 301 East Setauket,NY 11733 Southhold Building Department Attention: George Gillen RE: Request for Drainage Information on Building Permit# 30945: Address 13457 Oregon Road, Soja Residence. To whom it may concern, The 2' drainage grate located at the base of the driveway of 13457 Oregon Road and near the adjoining property is connected to three 8 foot wide by 12 foot deep drywells located under the front lawn. The drywells were installed several months ago by Artco Drainage. During the severe rain several weeks ago,the drain pipe in the 2' drainage pit leading to the drywells still had the initial installation duct tape (used to prevent dirt and sand from filling the pipe during installation)partially blocking the opening to the pipe. This caused the pipe to almost immediately clog when the rain started. The first work day following the flooding depicted in the pictures shown to me by Mr. Gillen, the grate was removed from the drainage pit,the drainage pipe was immediately cleared of debris and the installation tape was removed from the pipe opening. The pipe opening was also raised about eight inches off the bottom of the drainage pit so as to prevent any possible future clogging of the pipe opening. Since the repair to the pipe opening there have been no flooding problems whatsoever. I hereby certify as the Project Manager for the General Contractor, that I supervised both the installation of said drywells, as well as the repairs made to the drainage pipe opening subsequent to the date of the flooding depicted in the pictures shown to me by Mr. Gillen. Mr.Wolpert: We have recently received a copy of the Joint Application for Permit you filed with the New York State Department of Environmental Conservation(NYSDEC). If your proposal would involve such work, and has not been portrayed as such in your Joint Application, you should contact this office immediately so that a project-specific determination can be made as to whether a Department of the Army permit will be required. Any inquires should be directed to the Regulator-of-the-Day at 212-264-6731. Sincerely, L Michael G.
