This application must be filled in by typewriter or ink and submitted to the Building Department with the following:
A. For new building or new use:
I. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or
topographic features.
2. Final Approval from Health Dep!. of water supply and sewerage-disposal (S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifYing that the solder used in system contains less than 2/1 0 of I % lead.
5. Commercial building, industrial building, multiple residences and similar buildings and installations, a certificate
of Code Compliance from architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
B. For existing buildings (prior to April 9, 1957) non-conforming uses, or buildings and "pre-existing" land nses:
I. Accurate survey of property showing all property lines, streets, building and unusual natural or topographic
features.
2. A properly completed application and consent to inspect signed by the applicant. If a Certificate of Occupancy is
denied, the Building Inspector shall state the reasons therefor in writing to the applicant.

c. The work covered by 1his application may not be commenced before issuance of Building Pennit.
d. Upon approval oflbis application. the Building Inspector will issue a Building Permit to the applicant. Such a permit
shall be kept on lbe premises available for inspectioo lhrougbool lbe work.
c. No building shall be occupied or used in whole or in part for any purpose what so ever until the Building Inspector
issues a Certificate of Occupancy.
f. Every building permit shall expire if the work. authorized has not commenced within 12 months after the date of
issuance or has not been completed within 18 months:from such date. If no zoning amendments or other regulations affecting the
property have been enacted in the interim, the Building Inspector may au1horize, in writing, the extension of the permit for an
addition six months. Thereafter, a new permit shall be required.

DELIVERY TERMS AND CONDITION FOR SHEDS & GAZEBOS
Load Doors To I.Delivery is FREE for the first 30 miles from Kaufold's_ A chargeof$2.00 per mile is
charged for each additional LOADED mile. For buildings 12' wide the charge is $3.00
for each LOADED mile
2.Delivery is made to a level site prepared by the customer. Your required minimum access
" feet wide on straight back-drop (more access required if truck and trailer
must make turns) and minimum feet high. Customer is responsible to cut or
tie back tree limbs or bushes, removal of debris etc., to allow clearance.
3. Should obstructions cause a delay in delivery a charge 0[$24.00 per half hour will be
charged. Ifonit must be rolled an additional charge will apply.
4,Delivery of assembled buildings is made by a truck with an 8' wide, 26'long trailer. If low
hanging wires, trees, wet conditions, or fences prevent delivery, a return charge of
$75.00 will be charged to the customer.
5.Customer is responsible for obtaining any necessary required building permits and set back
requirements
6'oehveries are between 7:00 a.m, and 5:00 p,m. on your scheduled delivery day unless
otherwise notified. Ifcustomer is unavailable and truck must return with shipment
a $75.00 service charge will be applied.
7.0ur drivers are extremely careful when delivering a building, but we do not accept
responsibility for any damage to grass, shrubs, trees, etc.
