THIS INDENTURE, made the 1 (p+h day of m_ , 1995

State of New York, having its principal office at the Suffolk

WITNESSETH, that the party of the first part, pursuant to
Resolution Number 233-1995 adopted by the Suffolk County
Legislature on March 21 , 1995 and, thereafter, approved by the

and other valuable consideration paid by the party of the second
part, does hereby grant and release unto the party of the second
part, the heirs or successors and assigns of the party of the
second part forever,
