topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal (S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifYing that the solder used in system contains less than 211 0 of I % lead.
5. Commercial building, industrial building, multiple residences and similar buildings and installations, a certificate
of Code Compliance from architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
B. For existing buildings (prior to April 9, 1957) non-conforming uses, or buildings and "pre-existing" land uses:
I. Accurate survey of property showing all property lines, streets, building and unusual natural or topographic
features.
2. A properly completed application and consent to inspect signed by the applicant. If a Certificate of Occupancy is

a. This application MUST be completely tilled in by typewriter or in ink and submitted to the Building Inspector with 4
sets ot' plans, accurate plot plan to scale, Fee according to schedule.

c. The work covered by this application may not be commenced before issuance of Building Permit.
d. Upon approval ot' this application, the Building Inspector will issue a Building Permit to the applicant. Such a permit
shall be kept on the premises available for inspection throughout the work.
e, No building shall be occupied or used in whole or in part for any purpose what so ever until the Building Inspector
issues a Certificate of Occupancy.
f. Every building permit shall expire if the work authorized has not commenced within 12 months after the date of
issuance or has not been completed within 18 months from such date. If no zoning amertdments or other regulations affecting the
property have been enacted in the interim, the Building Inspector may authorize, in writing. the extension of the permit for an
addition six months. Thereafter, a new permit shall be required.

Gentlemen,
This letter is written to certify that the work described below has been executed in
accordance with the Building Code of the State of New York.
Scope of work (referenced drawiRl!:s)
Basement:
I. The crack in the east basement wall near the NE comer was grouted; the outside
of the wall received an application of water-proof paste that was protected by
waterproof felt before re-backfilling to grade.
2. The inner foundation wall was cut down approximately 12inches, 4 # 4 rebar 4in
oc was positioned and mass concrete was poured to fill gap between wall and the
main girder.
3. The two temporary Lally columns were replaced with permanent inverted 3in dia.
steel columns located in 2ft x 2ft x 2ft deep footings located 4ft below grade.
4. Conventional gal steel hangers were added to support all joists off the main
girders and foundation walls.
5. Slate shims were used to replace existing softwood shims at the locust post to
girder interfaces.
6. A fireproof drywall (Fire code Core Type X) ceiling was installed over the
furnace.
7. The CH an HW furnace burner was replaced.
First Floor
I. Y:. plywood and % hardwood floorings were installed over the existing
floorboards, except in the fireplace area where a :y. Masonite plate overlaid with :y.
tiles was used.
2. The doors to the sunroom were removed and an opening 12ft 7in wide was
formed by installing 3xl2in x 2in engineered (laminated) beams, end supported
by on 6 x 6 posts. The joints were reinforced by the addition of24in hurricane
straps nailed with 2 1I2in every nail-hole (approx 24 per strap) across each joint.
3. A model # SHR36A wood burning fireplace including glass doors was installed
on a ceramic slab. The associated prefabricated flue/combustion air induction pipe
and roof assembly were also installed following the manufacturer's minimurn
clearances were not violated. The fire brick enclosure was assembled and sealed
to the firebox.
4. A fireproof drywall enclosure was built around the fireplace and the enclosure
was extended to the roof of the living room where it was later sealed to the
cathedral ceiling, taking care to follow the manufacturer's required clearances.
5. Double doors in the sunroom west wall and all the windows in the entire house
were replaced with double hung windows. In the case of the sunroom the
openings were enlarged to accept slightly larger windows, 2x6 studs oc 16in and
2x12 headers were used in all cases. Between windows the studs were doubled
up. R19 was used in the sunroom walls that had been previously un-insulated.
6. An internal closet was built to house the new washer and drier, electrical, water
and drain service was added to these appliances.
7. A cathedral ceiling was formed in the living room by adding 2 x 9 collar beams oc
16in just below the ridge beam on each pair of existing rafters: and also 2 x 9
collar beams at an elevation 3ft below the fIrst set on approximately 4ft centers (to
accommodate the rafters). The rafter to collar beam joints used 5/8in bolts,
washers and nuts, two bolts per joint. Subsequently the attic floor joists were
removed and drywall and R19 insulation was installed before fInishing.
Exterior
1. A 24in x 26in timber and drywall enclosure was constructed to contain the
furnace flue assembly. A clearance of not less than 2in between the flue and the
enclosure was maintained.
2. The deck and stoop decking and top rails were replaced. Additional rails were
installed as required. Balusters were added on all rails. The gap between balusters
was maintained at less than 4in.
3. The existing wall cladding was replaced with natural cedar shakes.
4. CDX 1I2in plywood and ashfelt roofIng shingles were installed.
5. Supply 31 hurricane %in CDX shutters for the entire compliment of glazed
windows and door and the associated attachment hardware 3in hex bolts and % in
washers.
Other
1. Up-grade plumbing, install new well pump, water and drain services to washer.
2. Up-grade the electrical system, add outlets for washer, drier and smoke alarm
systems
3. Replace existing kitchen and bathroom cabinets, fIxtures and appliances.
4. The associated plumbing and electrical certifIcates have been issued.
