This application must be filled in by typewriter or ink and submitted to the Building Department with the following:
A. For new building or new use:
I. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or
topographic features,
2, Final Approval from Health Dept. of water supply and sewerage-disposal (S-9 form),
3, Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains less than 2/10 of I % lead.
5. Commercial building, industrial building, multiple residences and similar buildings and installations, a certificate
of Code Compliance from architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.

Dear Sir,
I have inspected the completed work for the above referenced property, Because the framing is covered by
wall and ceiling finishes I was not able to see the actual framing, During construction the owner made changes to
the original drawings as follows:
1, The front porch entry - The ceiling joists were not installed, For this small structure the need for ties is
not required,
2, The living room header was "upset" instead of being a dropped header as shown on the plan,
3, In the new addition two collar ties consisting of (3) 2x8 (per owner) instead of ceiling joists, The collar
ties will be adequate to hold th xterior wall from the expected lateral loads.

INSTRUCTIONS
a. This application MUST be completely filled in by typewriter or in ink and submitted to the Building Inspector with 4
sets of plans, accurate plot plan to scale. Fee according to schedule.
b. Plot plan showing location of lot and of buildings on premises, relationship to adjoining premises or public streets or
areas, and waterways.
c. The work covered by this application may not be commenced before issuance of Building Permit.
d. Upon approval of this application, the Building Inspector will issue a Building Permit to the applicant. Such a permit
shall be kept on the premises available for inspection throughout the work.
e. No building shall be occupied or used in whole or in part for any purpose what so ever until the Building Inspector
issues a Certificate of Occupancy.
f. Every building permit shall expire if the work authorized has not commenced within 12 months after the date of
issuance or has not been completed within 18 months from such date. If no zoning amendments or other reg-.uations affecting the
property have been enacted in the interim, the Building Inspector may authorize, in writing, the extension of the permit for an
addition six months. Thereafter, a new permit shall be required.
APPLICATION IS HEREBY MADE to the Building Department for the issuance of a Building Permit pursuant to the
Building Zone Ordinance of the Town of South old, Suffolk County, New York, and other applicable Laws, Ordinances or
Regulations, for the construction of buildings, additions, or alterations or for removal or demolition as herein described. The

Dear Mr. Wipf:
The Board of Town Trustees took the following action during its regular meeting held on
Wednesday November 17, 2004, regarding the above matter:
WHEREAS, ALEX WIPF applied to the South old Town Trustees for a permit under the
provisions of Chapter 97 of the Southold Town Code, the Wetland Ordinance of the
Town of Southold, application dated October 25,2004, and,
WHEREAS, said application was referred to the Southold Town Conservation Advisory
Council for their findings and recommendations, and,
WHEREAS, a Public Hearing was held by the Town Trustees with respect to said
application on November 17, 2004, at which time all interested persons were given an
opportunity to be heard, and,
WHEREAS, the Board members have personally viewed and are familiar with the
premises in question and the surrounding area, and,
WHEREAS, the Board has considered all the testimony and documentation submitted
concerning this application, and,
WHEREAS, the structure complies with the standards set forth in Chapter 97 of the
Southold Town Code,
WHEREAS, the Board has determined that the project as proposed will not affect the
health, safety and general welfare of the people of the town,

RESOLVED, that the Board of Trustees approve the application of ALEX WIPF to
construct a 10' x 38' extension to the existing dwelling and spread top soil from digging
the foundation 40' seaward from the existing house, behind a l' retaining wall, leaving
the area seaward of the retaining wall as a non-turf, non-fertilization buffer to the high
tide mark, place hay bales seaward of the construction of the wall, and all as depicted
on the plan surveyed by Van Tuyl and Son last revised May 13,1993, revised by Mr,
Wipf and approved by the Trustees on November 17, 2004.
Permit to construct and complete project will expire two years from the date the permit
is signed. Fees must be paid, if applicable, and permit issued within six months of the
date of this notification.
Inspections are required at a fee of $50.00 per inspection. (See attached schedule.)
This is not a determination from any other agency.

The Strong-Wall'" Shearwall can be installed around window and door openings, on garage
wing walls, interior waJls or any other locations where increased lateral resistance is required.
Strong-Wan~ Shearwalls can reduce the amount of wall space required for shearwalts,
allowing for more windows and doors in house designs.
Standard models are used for slab-an-grade applications where the wall will bear directly
on concrete. Simply fasten the bottom of the wall to the embedded bolts with the provided
