State Environmental Quality Review
FULL ENVIRONMENTAL ASSESSMENT FORM
Purpose: The full EAF is designed to help applicants and agencies determine, in an orderly manner, whether a project
or action may be significant. The question of whether an action may be significant is not always easy to answer. Frequent-
ly, there are aspects of a project that are subjective or unmeasureable. It is also understood that those who determine
significance may have little or no formal knowledge of the environment or may not be technically expert in environmental
analysis. In addition, many who have knowledge in one particular area may not be aware of the broader concerns affecting
the question of significance.
The full EAF is intended to provide a method whereby applicants and agencies can be assured that the determination
process has been orderly, comprehensive in nature,yet flexible enough to allow introduction of information to fit a project
or action.
Full EAF Components: The full EAF is comprised of three parts:
Part 1: Provides objective data and information about a given project and its site. By identifying basic project
data, it assists a reviewer in the analysis that takes place in Parts 2 and 3.
Part 2: Focuses on identifying the range of possible impacts that may occur from a project or action. It provides
guidance as to whether an impact is likely to be considered small to moderate or whether it is a potentially-
large impact. The form also identifies whether an impact can be mitigated or reduced.
Part 3: If any impact in Part 2 is identified as potentially-large, then Part 3 is used to evaluate whether or not the
impact is actually important.

D. Informational Details and Water Supply Narrative Request
1. Provide explanation of existing site use, occupancy, structures, sanitary flow,
water use; compare to proposed use, occupancy, structures, sanitary flow, water use.
The existing parcel is 27.15 acres of transitional vegetation representing change from Old-
field to shrub-land. The site may have been historically farmed or cleared for other
purposes. There are indications of former use as an orchard on the northerly sections of
the parcel.
The proposed use is a clustered subdivision for construction of twelve (12) single family
residential dwelling units and roadway on 13.69 acres with lot sizes that range between
40,000 and 55,955 square feet, access road from Main Road; and 13.46 acres of
agricultural use land for possible orchard use with a propose building (barn) envelop of
40,000 square feet. The zoning District is "AC." The proposed use is consistent with the
existing zoning for this district. Water use and sanitary calculations are estimated as:
Sanitary Discharge Flows (single family residential dwellings as estimated from Suffolk
County Department of Health Services)
Residential:
300 gallons per day per dwelling x 12 units=3,600 gallons per year.
Water Use (single-family residential dwellings as estimated from Suffolk County
Department of Health Services sanitary flow with an addition of 15% factor.
3,600 gpd plus 15% =4140 gpd. Or 1,511,100 gallons per year.
The Orchard/Vineyard:
Water use for orchards or vineyards is generally less than the 140,000 gallons per acre of
agricultural land as predicted by the Suffolk County Department of Health Services
(1983). However the orchard or vineyard use would be expected to require less water
than other typical vegetable crops or sod.
Orchard and vineyard crop types can be watered with sub-surface deep or drip type
irrigation systems that reduce loss by evaporation. Vineyards require 620 gpd for the first
year after planting and 1,860 per day after that(Mudd 1997). Based on vineyard use as a
model, the agricultural piece is predicted to require (for a growing period of 34 weeks)
1,860 gpd x 13.46 acres=24,180 gpd (or 5.754840 MG per year: 17.65 acre-feet of water
per year).

Residential Irrigation:
Residential irrigation is based on 1" per water per acre of landscape area per week for 34
weeks of seasonal growing period. For the proposed development it is estimated at:
(13.69 acres— 1.0 acre of roadway) x 20% building coverage (assumed) = 9.952 acres of
landscaped area x 0.083 ft irrigation water per week = 0.823 acre-feet per week x 34
weeks= 28.08 acre-feet.
Or 0.118 acre-feet per day during the irrigation season. One-acre foot of water is
approximately 326,000 gallons of water and the total daily irrigation is estimated at
38,330 gallons of water or 3,200 gallons per unit per day.
Water use is estimated to be 42,640 gallons per day for an average "irrigation day" for
the entire subdivision. 42,640 gpd/12 units = 3,553 gpd/unit @ 10 gpm pumping rate
(assumed small capacity well pump) is 355.3 minutes of pumping per day or 6 hours per
day pumping rate for each household.
The site does not currently require irrigation.
2. Indicate the source of water supply, nearest public water main, nearest public
well field, and adjacent private wells (if known).
The proposed source for water is from individual private on-site drinking water wells in
conformance with Article 4 of the Suffolk County Health Department "Private Water
Systems". The nearest public water main is located along the north side Main Road
(Route 25) and the nearest public well field is at Ackerly Pond Lane and on Jasmine
Lane/Blossom Lane east of the site, where a residential subdivision is provided public
water.
3. If public water supply is proposed, indicate the ability of the water utility to
provide water supply to the project. Provide a letter of availability or detailed
explanation of status of review by water utility.
Public water is unknown accessible from the perspective of mains located near the
subject site. Availability of the water is unknown. The applicant will seek Suffolk County
Department of Health Services approval for on-site private water supply wells. The
County or another involved agency may require the applicant connect to municipal water
supplies and therefore this statement will need to be modified. It is not the intent of the
applicant to seek connection to public water supply.
4. If private water supply is proposed, indicate the well specifications, water quality
based on site-specific water quality data.
The applicant will provide additional information on well specifications and site-specific
water quality data as required by the Suffolk County Department of Health Services
Article 406 Standards for Private Water Systems. Test wells will be isolated in
accordance with 406-12 (a) General Requirements for Test Wells and (b) Specific Test

Well Requirements for Realty Subdivisions. Upon direction by the Health Department to
install test well(s) and conduct tests the applicant's representative(s) will submit it to the
Planning Board as an addendum to this submission.
5. Indicate proposed water conservation measures that will mitigate for unavoidable
adverse impacts (If any). Conservation measures should include, but not be limited to.
The use of drought tolerant plants for landscaping.
The applicant has not developed the landscape plans for the project because the project is
only in the stage of subdivision review. The applicant will (if directed by the planning
Board)consider the use of native plant species that are drought tolerant.
The preservation of existing native vegetation.
Existing vegetation is a collection of old-field and successional shrub-land; some of these
species (Eastern Red Cedar for example) can be preserved. Other species such as
bittersweet should be eliminated. The applicant cannot develop an adequate preservation
plan until the subdivision review process better defines the lot lines and proposed
building footprints.
The conservation of existing open space areas.
The applicant has provided a cluster subdivision that includes more than 13 acres
(approximately 50% of the entire parcel) as open space with possible use as agriculture.
The applicant proposes to preserve the existing native vegetation within this area until a
more define agricultural use is known.
The implementation of"natural" storm water management practices (grass swales,filter
strips, created wetlands, etc.)
The applicant will utilize methods for "natural" storm water management controls
including grassed swales, and filter strips. These decisions will be developed during the
subdivision and site plan review process.

Dear Mr. Cuddy:
Water supply and quality within the Town of Southold is a serious community issue. Public water supply,
although provided by the Suffolk County Water Authority, is determined through the current and
projected use of the community. To date, the supply does not meet the demand,however,water use is
controlled by an individuals' actions and can often be mitigated through numerous water conservation
measures implemented during the planning process.
Water quality is very difficult to control. Although point source pollution can usually be traced to its
origin, non-point source pollution is boundless and one of the largest threats to local environmental and
community health. Non-point source pollution has been identified as the primary cause of water quality
degradation in more than 90 percent of New York State's impaired water bodies. In the Town of
Southold, localized well and surface water contamination already exists.
Consequently, the Town of Southold Planning Board in cooperation with the Suffolk County Water
Authority now requires the completion and submittal of the attached Environmental Assessment Form,
Section D. Informational Details and Water Supply Narrative as part of the SEQR review process. Until a
completed Environmental Assessment Form, Section D. Informational Details and Water Supply
Narrative is submitted, applications will be considered incomplete and the SEQR review will not proceed.

subdivision was discussed and re-discussed for months at a time.
It is my understanding that it complies with your requirements and
I respectfully request that you advise when this applicant will
receive sketch plan approval.
I am aware, as is the applicant, that the Town has been without an
individual in the Planning Department to review subdivision maps.
However, this subdivision was reviewed prior to that time, i.e. ,

In regard to the Zoumas subdivision, I would wait on making any changes to the map
until I inspect the site with the Trustees next week. The current map may be acceptable,
or we may have a better idea of the best layout.
In regard to Flower Hill, I am confirming the three items that we asked for at the work
session. The cluster sketch plan should be amended by moving the building envelope on
the large lot towards the comer with the wetlands. You may want to leave more room
than the 100' minimum setback. The cul-de-sac can also be shortened, but the Planning
Board does not want long flag lots. A yield plan should also be submitted to make sure
that you can get all of the lots proposed in the sketch plan.

Dear Mr. Cuddy,
I have examined the maps that you submitted for the above referenced subdivision. I
• have noticed the following problems with the yield map:
1. Three of the flag lots are invalid lots. The area of the"flagpole" is not allowed to
be counted toward the area of the lot. Therefore, lots 4, 6, & 7 are less than
80,000 sq. ft.
2. There are too many flag lots on the map. This design would not be an acceptable
subdivision layout.
3. The area fronting Main Road is zoned RO, not B.
I have also noticed the following problems With the cluster sketch map:
1. There are still two illegal lots on Main Road. This space should probably be
connected with the neighboring lots and the Planning Board may request a
conservation buffer zone along the road. The Board is not in favor of a gate
house on this property.
2. The fire lane is shown as a separate parcel. This should be discussed with the
Planning Board. It was originally discussed that the fire lane would be a hard
surface covered with grass, and shown as a right-of-way shared between the two
neighboring parcels. The Town will not want to maintain a grass area. If this is
being proposed as a separate parcel, the Town may require paving and dedication
as a Town road.
3. The Board will be requiring a building envelope on the large lot. The location
• should be discussed With the Board.
4. The A-C/RO zoning line should be shown on the map.
• 5. At least one lot should show the numerical setbacks of the building envelope.
6. The map should be described as a"cluster sketch map".
The general lot and road layout of the cluster sketch plan is acceptable to the Planning
Board.
