THIS IS TO CERTIFY THAT THE FOLLOWING RESOLUTION NO. 2006-796 WAS
ADOPTED AT THE REGULAR MEETING OF THE SOUTHOLD TOWN BOARD ON
OCTOBER 3, 2006:
RESOLVED that the Town Board of the Town of South old hereby authorizes and directs
Supervisor Scott A. Russell to execute an ae:reement entitled "Statement of Work" between
the Town of Southold and Verizon Network Intee:ration regarding the wireless project for the
Police Department, subject to the approval of the Town Attorney.

This Statement of Work ("SOW") is by and between Verizon Network Integration ("Verizon") and the Town Of South old
("Customer") as of the date last written below ("February 24, 2004") and is governed by the terms and made a part of the
System Agreement ("Agreement") between Verizon and Customer.
This SOW defines the services and deliverables that Verizon shall provide to Customer under the terms of the Agreement
("Services"). This SOW also defines the responsibilities ofVerizon and the Customer, the project scope, implementation
duration, installation acceptance procedures, and Change Control Procedures. Verizon will perform the defined services at
the locations listed in Exhibit A. Verizon will strive to meet Customer's schedule requirements; however, actual project
dates will he subject to availability of material and resources.
Verizon will use reasonable efforts to avoid interruption of network service unless the Customer requires installation after
hours. If it is necessary to interrupt network service, however, Verizon will notify the Customer SPOC at least 48 hours in
advance.
Project Scope
. The scope of this project includes the installation of Cisco Airouet 1300 wireless access points with an external
Omni directional antenna, four (4) Cisco switches, and one (I) Cisco router to 4 previously designated locations.
Wireless AP Integration:
Verizon's will survey each of the 4 installations locationslbuildings for the best location for attachment of each
of the wireless units to ensure maximum coverage. Verizon will pre-configure all project wireless equipment in
and verify all equipment is functional and configured to customer specification prior to being sent to each of the
4 locations to be physically installed. Verizon will also run CA TSe Ethernet cables from the chosen installation
points to the appropriate/nearest data closet within the appropriate building. Once the physical Access Point
installation is complete Verizon will install a pc wireless card into 2 to 3 patrol cars per location to verify
network access from vehicles to each wireless Access Point. Verizon will also execute a detailed verification
check list to ensure the design and the overall installation has met the client's needs.
1.lKey Assumptions
Pricing is hased on the following key assumptions:
. Verizon performs installations between the hours of8:00 a.m. and 5:00 p.m local time Monday through Friday,
excluding Verizon observed and Federal holidays. Off-hours are defined as anything other than those hours. This
project does not require off-hour work.
. Customer will provide a (Single Point Of Contact) "SPOC" to be assigned to this project who will have the skills
necessary to assist V erizon in this project
. Pre-configuration of all project wireless equipment and verify all equipment is functional and configured to
customer specification prior to being sent to Job locations at each of the 4 locations to be physically installed.
. A fmal site survey and inspection with customer SPOC will be done prior to installation at each of the 4
installations locationslbuildings for the best location for attachment of each of the units to ensure maximum
coverage and power is available.
. The project is to take place over a 4 day time-frame
. Verizon assumes that the 802.lIG equipment to be installed will not interfere with existing patrol car
equipment.

Verlzon Responsibilities
a Provide a Single Point of Contact (PM- Project Manager) to manage, and participate in the kickoff discussion,
schedule coordination, and acceptance testing.
a The Verizon PM will contact Customer prior to installation in order to confirm site readiness and schedule
equipment delivery.
a Veriron will perform a Final Site Survey per Location
a Verizon will be responsible for any roof penetrations for cable root into building for only the Hospital location
( Eastern Long Island Hospital Access Point installation) for example: a soflt
Exterior Site survey tasks performed:
. Determine ideal location for Patrol Car wireless access.
a Measurement of the signal strength between Building and Assigned Patrol Car area.
. Determine best location for the mounting of external Wireless radio (ex: building, mast).
Wireless Network Site Assessment Report:
a Documentation of the requisites for a Wireless Local Area Network (WLAN) implementation.
a Installation specification document.
a Analysis of the survey's results and a network design providing for your desired coverage.
a Plans created for integration of the wireless LAN into the existing network.
a Details of all required cbanges to existing network.
a Equipment placement.
a Power considerations and wiring requirements.
. Logical diagram for the proposed WLAN.
a A listing of all facility, bardware, and software prerequisites.
Equipment Staging and ConfJgUr8tion
a Verify system power-up, operation of network interfaces and run internal diagnostics.
a Configure Access Points as per design specified in Site Surveys.
a Deliver Equipment to each location for installation
a Configure ssm.
a Configure Security.
a Configure client card configuration
. Document network equipment configuration, as per pre-sale negotiations, and provide a written copy to Customer.
a Verizon will also run CA T5e Ethernet cables from the chosen installation points to the appropriate/nearest data
closet within the appropriate building
a Once the installation of the 4 Access points are installed, and configured, Verizon will instal! a pc wireless card
into 2 or 3 patrol cars per location to verify network access from vehicles to each access point.
a Verizon will provide one day of laptop configuration.
a Verizon will execute a detailed verification check list to ensure the design and the overall installation has met the
customers needs
a Verizon will be providing all hardware and services as listed in Quote # 8-25V2B8
Equipment Installation and Integration
. Verizon will provide one day of laptop configuration support for the southold SPOC

. Verizon will test connectivity of laptops to wireless network.
. Verizon will install 10 wireless client cards in patrol cars for connectivity testing to Precinct.
. The Southold PO will install wireless cards in the remainder of the laptops.
Skills Transfer
. During the course of the project Verizon will instruct the designated Southold PO personnel on the
configuration and support of the new devices and wireless system.
1.3. Customer Responsibilities
. Provide a Single Point of Contact (SPOC) to resolve implementation issues and to participate in the kickoff
discussion, schedule coordination, and acceptance testing. Customer shall provide the name and contact
information of such individual to Verizon in writing.
. Assume responsibility for hardware, software and memory compatibility issues related to existing equipment.
. Prior to installation, perform back up of any involved device, including drivers, applications, and operating
systems as required
. Be responsible for providing licensed copies of operating system and applications software should they need to be
re-installed during the installation, as applicable. Customer is responsible for installation or re-installation of any
software not provided by Verizon on this project.
. Control all activities associated with the existing cnstomer equipment, including changes, additions or deletions of
devices made by any non- Verizon provided technicians.
. Provide prompt physical and electrouic access to all areas/devices where Verizon will install equipment. NOTE:
Wait time in excess of30 minutes may result in a time and material charge.
. Customer is responsible for troubleshooting existing network components and/or client
Workstations; to ensure all of the customer LAN! Wan networking components, and equipment necessary are
operational at time of project implementation
. Customer is responsible for any roof penetrations for cable root into building for the police station, Recreation
Building, and the North Fork Bank Building; for example: a sofit
1.1.4. Change Order Request
Verizon will provide Customer with a Verizon Change Order Request form, substantially in the form set forth in Exhibit B.
Customer may request changes in, or additions to, the work being provided hereunder by completing the Verizon Change
Order Request form. Requested changes will be facilitated to the extent feasible. If V erizon determines that such changes
will cause an increase or decrease in the cost of or time required for performance of the work, Verizon will advise
Customer thereof and such adjustments shaH be reflected in the Verizon Change Order Request form. The Verizon Change
Order Request form shall not become effective unless and until it is agreed to and executed by Customer and Verizon.
Verizon will initiate changes to the project that affect cost or significantly affect schedule using the Change Control
procedure.

Payment terms should not be included in the SO W unless they are different than what is set forth in the
Agreement

1.6. Acceptance Testing
Verizon and Customer will perform acceptance testing at the time of installation. If Customer fails to provide access for
Verizon to perform acceptance testing, or fails to participate in acceptance testing within 5 business days of being notified
by Verizon that a site is ready for acceptance, the site(s) shall be deemed accepted by Customer and will be invoiced as
though acceptance testing had been actually performed. A Customer signsture on the Certificate of Acceptance, as set forth
in Exhibit C or other Verizon standard acceptance documents, signifies that all deliverables have been completed for the
site. Verizon and Customer acknowledge and agree that this SOW accurately sets forth the Services that Verizon will
provide to Customer, and is signed by an authorized representative of Customer and Verizon.
