Dear Dr. Notaro:
The following resolutions were adopted by the Southold Town
Planning Board at a meeting held on Monday, July 16, 1990.
RESOLVED that the Southold Town Planning Board, acting
under the State Environmental Quality Review Act, make a
determination of non-significance, and grant a Negative
Declaration.
WHEREAS, Antoinette Notaro, M.D. is the owner of the
property known and designated as Antoinette Notaro, M.D.
SCTM#1000-64-1-18, located at 170 Mechanic Street, Southold; and
WHEREAS, a formal application for the approval of this site
plan was submitted on June 28, 1990; and
WHEREAS, all the requirements of the Site Plan Regulations
of the Town of Southold have been met; and

RESOLVED, that the Southold Town Planning Board grant
conditional final approval on the surveys dated July 11, 1990,
and authorize the Chairman to endorse the final surveys subject
to fulfillment of the following conditions. All conditions must
be met within six (6) months of the date of this resolution.

Certification by the Building Department.
Acceptance by the Planning Board of a landscape plan
showing buffer plantings between parking area and
south property line and the retention (to the extent
possible) of buffer vegetation on the north property
line.
Enclosed please find a copy of the Negative Declaration for
your records.

Enclosed is the site plan application which we ha~
discussed.
The reason for this submission is to change the use of
this building. It is presently used as a residence, and I
would like to use it for my office. I believe this is an
an approved use in this Hamlet Business zone. The existing
structure would be improved but essentially remain the same.
The rear portion° which is a one story addition, will need to
be replaced due to structural faults.

4.6 PARKING SPACES AND PASSENGER LOADING ZONEs
4.$ Ground and Floor Surfaces
4.5.1' General. (iround and floor surfaces along
accessible routes and in accessible rooms and
spaces, including floors, walks, ramps, stairs, and
curb ramps, shall be stable, firm, and slip resistant,

4.5.2 Changes in Level. Changes in level up to ~

4.5.3* Carpet. If carpet or carpet tile is used on a
ground or floor surface, then it shull be securely
attached; have a firm cushion, pad, or backing or
no cushion or pad; and have a level loop, textured
loop, level cut pile, or level cut/uucut pile texture.
The maximum pile height shall be % in (13 mm).
Exposed edges of carpet shall bc fastened to floor
surfaces and have trim along the entire length of
the exposed edge. Carpet edge trim shall comply
with 4.5,2.
4.5.4 Gratings. If gratings are located in walking
surfaces, then they shall have spaces no greater
than ~ in (13 mm) wide in one direction. Il'grat-
ings have elongated openings, then they shall be
placed so that the long dimension is perpendicular
[o the dominant direction of travel.
4.6 Parking Spaces and Passenger Loading
Zones
4.6.1 General. Accessible parking spaces shall
comply with 4.6.2. Accessible passenger loading
zones shall comply with 4.6.3.
