Please note that inasmuch as the building is existing and will not be altered, the landscaping which presently exists in the front yard will remain undisturbed where possible. The lawn will be replanted in the area of the new septic system. The proposed plantings which flank the east and west side of the new parking area in the rear yard have been shown as a 5' high arbor-vitae hedge. The existing plantings and lawn south of the new parking area will remain undisturbed where possible or be restored with grass. Regarding the exterior ligting, the plan shows a building mounted residential type 300 watt quartz flood light to provide illumination for the parking area. In addition there will be building mounted residential type 75 watt PAR flood lights on the west and north side of the building for direct illumination of the ramp and entry areas only. This type of fixture would be typically found in this residential setting and can be adjusted so as to control the areas to be illuminated. I approval 19th. INSTRUCTIONS FOR FINAL A~PROVAL OF CONSTRUCTED SYSTEMS It is the applicant's responsibility to call the Department 24 hours in advance to arrange inspections of the sanitary system and water supply facilities prior to backfilling. This includes inspections of the soil excavation for the sanitary system (when necessary) and inspections of the water supply well, well lateral, public water supply line, sanitary system and piping. As-built plans must consist of at least the following: (1) the lot location, dimensions, elevations and lot number of subdivision (if applicable); (2) the exact location of the private well or public water line (give at least 2 dimensions to the well head measured from the corners of the building); (3) the exact location of the sanitary disposal system including septic tank, leaching pool(s) and other a~purtenances (give at least 2 dimensions measured from the corners of the building); (4) the exact location of all sewer lines and the sewage treatment plant (if applicable); {5) a floor plan of the building showing usage of each unit and identification by letter or number of all units (if applicable}; (6) for denitrification systems, include invert elevations at inlets and outlets of all units in system and piping manifold; and (7) have a clear area at least 3" x 5", directly above the title block, for the DepartmentVs appro'.~l sta~.:.. 2. Applications must comply with all relevant density and construction standards for water supply and sewage disposal. Where applicable, the depa~iment regulates the storage, handling and discharge of restricted toxic and hazardous materials pursuant to the requirements of Articles 7 & 12 of the SCSC. Project designs submitted to the department should be fully consistent with any action currently under review through the SEQRA process. Design and flow specifications, information regarding subsurface soil conditions, water supply information, and complete design details are essential to the review of this project, and are evaluated fully at the time of application review. Should the town require additional environmental information (such as a DEIS, or EAF Part III) discussion of the compliance requirements of the SCDHS should be required. Of particular concem to the departmem are those areas which because of elevated groundwater conditions, or soils which are not conducive to the proper functioning of conventional subsurface sanitary sewage disposal systems. Your agency should be aware that such conditions frequently require the use of fill or the excavation of subsurface soils to accommodate subsurface sanitary disposal systems constructed in conformance with the requirements of the SCSC. The department is also significantly concerned with areas where access to potable water may be constrained by unacceptable groundwater quality and the lack of an available public water supply. All private water supply systems must be constructed in conformance with requirements of the SCSC. H. NATURAL RESOURCES: The SCDHS fully supports all efforts to maximize protection of natural resources which may be impacted upon by construction and development activities. It is the position of the department that the SEQRA review process provides the greatest opportunity for comprehensive consideration of these resoumes, and that all practicable planning measures should be employed to help ensure their protection. Of particular concern to department is the adequate protection of wetlands, surface waters, natural communities, contiguous natural habitats, and rare, threatened and endangered species. In addition, efforts to protect sensitive physical resources such groundwaters, dunes, bluffs, shorelines, natural drainage channels, groundwater recharge areas, and steep slopes are fully supported and encouraged by the SCDHS. Construction of recharge areas, so as to minhnize the amount of disturbance and structural modification to the site 8. Maximum use of "land-banked" parking on commercial sites 9. Minimal use of fertilizer-dependant turf and landscaping 10.
