Dear Mrs. Audion:
Please be advised that upon the recommendation of the Town
Board, the Planning Board is now going to forward all minor
subdivisions to the Board of Health for their approval prior
to the Planning Board granting final approval on the minor
subdivision. This action has received the endorsement of
the Health Dept. and it should expediate building permits
on lots in these minor subdivisions.
This procedure will pertain to your proposed minor subdivision
located at W/S Marratooka Road, Mattituck, N.Y.
Please submit six (6) copies of map containing all information
as listed on the "Suffolk County Health Dept. Procedure and
Requirements for Approval to Construct on Individual Lots"-
(green sheet enclosed)
Enclosed, please find three (3) copies of Suffolk County Health
Dept. form "APPLICATION FOR APPROVAL OF SANITARY FACZLITIES FOR
REALTY SUBDIVISIONS." Please complete these forms and sign them,
then forward them to your surveyor for his completion an~$ignature.
These forms must be completed in full, all questions must be
answered. The forms must then be returned to us, at which time
we will forward them, together with the maps, to the Health Dept.
