The following resolution was adopted by the Southold Town Planning Board
at a meeting held on Monday, October 3, 1994:
BE IT RESOLVED that the Southold Town Planning Board authorize the
Chairman to endorse the final sur~eys dated July 50, 1995. Conditional final
approval was granted on June 15, 1994. All conditions have been fulfilled.
Enclosed please find a copy of the map which was endorsed by the
Chairman. The mylar maps, which were also endorsed by the Chairman,
must be picked up at this office and filed in the office of the County Clerk.
Any plat not so filed or recorded within sixty (60) days of the date of final
approval, shall become null and void.

THIS DECLARATION made the

PECONIC HOMES CORPORATION, a domestfc corporation with offices
located at (no #) Sound Ave., Mattituck, N9 11952, hereinafter
referred to as the Declarant, states as follows:
WHEREAS, Declarant is the owner of certain real property
situate in the Town of Southold, Suffolk County, New York, more
particularly bounded and described as set forth in Schedule #A"
annexed hereto as provided by the Declarant; and
WHEREAS, for and in consideration of the granting of a
subdivision application entitled, "Minor Subdivision Plat for
Peconic Homes, Inc." and by Resolution dated October 5, 1993, a
copy of which is attached hereto and made a part hereof, and as a
condition of granting said approval, the Town of Southold Planning
Board has required that the within Declaration be recorded in the
Suffolk County clerk's office; and
WHEREAS, Declarant has considered the foregoing and
determined that same will be in the best interest of the Declarant
and subsequent owners of said parcel;
NOW, THEREFORE, THIS DECLARANT WITNESSETH:
That Declarant, for the purpose of carrying out the
intentions above expressed does hereby make known, admits,
publishes, covenants and agrees that the said premises herein
described shall hereafter be subject to the following covenants
which shall run with the land and shall be binding upon all
purchasers and holders of said premises, their heirs, executors,
legal representatives, distributees, successors and assigns, to
wit:
(A) Prior to the issuance of any building permit for any
dwelling unit on proposed Lot Number 1, such apDlication shall be
reviewed by the Planning Board. The Planning Board shall
determine whether the additional impact warrants inclusion of the
action with the June 7, 1989 Positive Declaration or requires any
further environmental review.
(B) Prior to the issuance of any building permit for any
dwelling unit on proposed Lot Number 1, such application shall be
reviewed by the Planning Board. The Planning Board shall
determine what road improvements are necessary due to the impact
of the additional dwelling unit.
(C) A park and playground fee will be required at such time
that a dwelling unit is allowed on Lot #1.
(D) These covenants and restrictions can be modified only at
the request of the then owner of the premises with the approval of
a majority plus one of the Planning Board of the Town of Southold
after a public hearing. Adjoining property owners shall be
entitled to notice of such public hearing, but their consent
tosuch modification shall not be required.

Purpose: The full I!AF is designed to help applicants and agencies determine, in an orderly manner, whether a project
~)yr, action may be significant. The question of whether an action may be significant is not always easy to answer. Frequent-
there are aspects of a project that are subjective or unmeasureable. It is also understood that those who determine
significance may have little or no formal knowledge of the environment or may be technically expert in environmental
analysis. In addition, many who have knowledge in one particular area may not be aware of the broader concerns affecting
the question of significance.
The full F^F is intended to provide a method whereby applicants and agencies can be assured that the determination
process has been orderly, comprehensive in nature, yet flexible to allow introduction of information to fit a project or action.
Full £AF Components: The full EAF is comprised of three parts:
Parl 1: Provides obiective data and information about a given project and its site. Dy identifying basic project
data, it assists a reviewer in the analysis that takes place in Parts 2 and 3.
Part 2: Focuses on identifying the range of possible impacts that may occur from a proiect or action. It provides
guidani:e as to whether an impact is likely to be considered small to moderate or whether it is a potentially*
large impact. The form also identifies whether an impact can be mitigated or reduced.
Part 3: If any impact in Part 2 is identified as potentially-large, then Part 3is used to evaluate whether or not tile
impact is actually important.

Purpose: The full £AF is designed to help applicants and agencies determine, in an orderly manner, whether a project
or action may be significant. The question of whether an action may be significant is not always easy to answer. Frequent-
ly, there are aspects of a project that are subiective or unmeasureable. It is also understood that those who determine
significance may have little or no formal knowledge of the environment or may be technically expert in environmental
analysis. In addition, many who have knowledge in one particular area may not be aware of the broader concerns affecting
the question of significance.
The full EAF is intended to provide a method whereby applicants and agencies can be assured that the determination
process has been orderly, comprehensive in nature, yet flexible to allow introduction of information to fit a project or action.

PECONIC HOMES CORPORATION, a domestic corporation with offices

referred to as the Declarant, states as follows:
WHEREAS, Declarant is the owner of certain real property
situate in the Town of Southold, Suffolk County, New York, more
particularly bounded and described as set forth in Schedule "A"
annexed hereto as provided by the Declarant; and
WHEREAS, for and in consideration of the granting of a
subdivision application entitled, "Minor Subdivision Plat for
Peconic Homes, Inc." and by Resolution dated October 5, 1993, a
copy of which is attached hereto and made a part hereof, and as a
condition of granting said approval, the Town of Southold Planning
Board has required that the within Declaration be recorded in the
Suffolk County clerk's office; and
WHEREAS, Declarant has considered the foregoing and
determined that same will be in the best interest of the Declarant
and subsequent owners of said parcel;
NOW, THEREFORE, THIS DECLARANT WITNESSETH:
That Declarant, for the purpose of carrying out the
intentions above expressed does hereby make known, admits,
publishes, covenants and agrees that the said premises herein
described shall hereafter be subject to the following covenants
which shall run with the land and shall be binding upon all
purchasers and holders of said premises, their heirs, executors,
legal representatives, distributees, successors and assigns, to
wit:
(A) Prior to the issuance of any building permit for any
dwelling unit on proposed Lot Number 1, such aDDlication shall be
reviewed by the Planning Board. The Planning Board shall
determine whether the additional impact warrants inclusion of the
action with the June 7, 1989 Positive Declaration or requires any
further environmental review.
(B) Prior to the issuance of any building permit for any
dwelling unit on proposed Lot Number 1, such application shall be
reviewed by the Planning Board. The Planning Board shall
determine what road improvements are necessary due to the impact
of the additional dwelling unit.
(C) A park and playground fee will be required at such time
that a dwelling unit is allowed on Lot #1.
(D) These covenants and restrictions can be modified only at
the request of the then owner of the premises with the approval of
a majority plus one of the Planning Board of the Town of Southold
after a public hearing. Adjoining property owners shall be
entitled to notice of such public hearing, but their consent
tosuch modification shall not be required.

Dear Mr. Abruzzo:
The following resolutions were adopted by the Southold Town
Planning Board at a meeting held on Monday, October 4, 1993:
WHEREAS, a major subdivision application for the Peconic Homes
property has been before the Planning Board since 1988 and has
not proceeded due to the applicant's decision to not proceed in
response to the SEQRA Positive Declaration issued by the
Planning Board on June 7, 1989; and
WHEREAS, the applicant has now applied for a minor subdivision
to create a building lot with an existing house in order to
separate the use from the overall property which is the subject
of the previously mentioned major subdivision; and
WHEREAS, the Planning Board currently has before it two
different applications for the above mentioned parcel; one for a
major subdivision, and the other for a minor subdivision; and
WHEREAS, the Board can not proceed simultaneously with two
different applications for the same parcel, its review of the
major subdivision will be put on hold; and
WHEREAS, the intent of the minor subdivision application is to
separate the existing dwelling (shown as proposed Lot Number 2
on the minor subdivision map dated July 30, 1993) from the total
45.97 acre parcel; and
WHEREAS, Lot Number 1 (the remaining 43.5784 acre parcel shown
on the minor subdivision map dated July 30, 1993), which
currently contains a mobile home and numerous greenhouses,
not be improved with any additional dwelling units unless
certain conditions are met; and

Be it therefore RESOLVED that the Southold Town Planning Board
grant sketch approval on the map dated July 30, 1993 subject to
the following condition:
A Declaration of Covenants and Restrictions including
the following terms must be recorded prior to any
approval of the subdivision:
A. Prior to the issuance of any building permit for
any dwelling unit on proposed Lot Number 1, such
application shall be reviewed by the Planning
., Board. The Planning Board shall determine whether
the additional impact warrants inclusion of the
action within the June 7, 1989 Positive Declaration

Sketch plan approval is conditional upon submission of final
maps within six months of the date of sketch approval, unless an
extension of time-is requested by the applicant, and granted by
the Planning Board. The final maps (2 mylars and 5 paper
prints) must contain a current stamp of Health Department
approval, and must be submitted before a final public hearing
will be set.
The sketch plans have been referred to the Suffolk County
Planning Commission for review. You will be notified upon
receipt of the report from the Commission. The Planning Board
may require additional covenants and restrictions upon review of
the report. Therefore, it is not necessary to prepare the'draft
Declaration of Covenants and Restrictions at this time.
The sketch plans have also been referred to the Mattituck Fire
District for their recommendation as to whether a firewell is
necessary for fire protection.

subdivision plan, plea.se be aware that subdivision of land/a not regulated under the
Freshwater Wetlands Act, Article 24 of thc New York State Environmental Conservation Law.
Therefore no frmhwater wetlands permit is needed for the act of subdivision.
As per Article 24 any improvements to the property, including clearing, grading, excavation,
filling, or construction require a permit if performed within 100 feet of the NYSDEC Freshwater

HIS DECLARATION made by PECONIC HOMES CORPORATION this
-~ day of March, 1994, a domestic corporation with offices
located at (no $) Sound Ave., Mattituck, New York 11952 hereinafter
referred to as the DECLARANT, as the owner of premises described in
Schedule "A" annexed hereto (hereinafter referred to as the
PREMISES) desires to restrict the use and enjoyment of said PREMISES
and has for such purposes and determined to ~mpose on said PREMISES
covenants and restrictions and does hereby declare that said
PREMISES shall be held and shall be conveyed subject to the
following covenants and restrictions:
WHEREAS, DECLARANT has made application to the Suffolk
County Department of Health Services (hereinafter referred
to as the DEPARTMENT) for a permit to construct and/or
approval of plans for a single family residence, a
subdivision or development or other construction project on
the PREMISES; and

2. WHEREAS, the PREMISES are to be served by an individual on-
site private well; and
WHEREAS, the test wells sampled for the PREMISES indicated a
groundwater supply that had contamination in excess of the
minimum drinking water standard and/or guidelines of the
State of New York and contained excess of the following:

A complete list of analysis results is attached hereto as
Schedule C, and each chemical parameter which exceeds 60% of
the acceptable level is.indicated with an asterisk; and
WHEREAS, the County of Suffolk Department of Health Services
has agreed to issue a permit only if there be a record
covenant that the necessary water conditioning equipment be
installed so as to meet the quality standards for drinking
water; it is DECLARED and COVENANTED by DECLARANTS, their
heirs or successors and assigns forever, that no residence
upon the above described property will be occupied prior to
the installation of the necessary water conditioning
equipment so that the water, when conditioned, meets the
said minimum quality standards for drinking water of the
State of New York and evidence of the same is furnished to
the Suffolk county Department of Health Services for their
written approval.

The DECLARANT, its successors and/or assigns shall set forth
these covenants, agreements and declarations in any and al
leases to occupants, tenants and/or lessees of the above
described property and shall, by their terms, subject same
to the covenants and restrictions contained herein. Failure
of the DECLARANT, its successors and/or assigns to so
condition the leases shall not invalidate their automatic
subjugation to the covenants and restrictions.
Ail of the covenants and restrictions contained herein shall
be construed to be in addition to and not in derogation or
limitation upon any provisions of local, state and federal
laws, ordinances, and/or regulations in effect at the time
of execution of this agreement, or at the time such laws,
ordinances, and/or regulations may thereafter be revised,
amended or promulgated.
This document is made subject to the provisions of all laws
required by law or by their provisions to be incorporated
herein and they are deemed to be incorporated herein and
made a part hereof, as though fully set forth.
The aforementioned Restrictive Covenants shall be
enforceable by the County of Suffolk, State of New York, by
injunctive relief or by any other remedy in equity or at
law. The failure of said agencies or the County of Suffolk
to enforce the same shall not be deemed to affect the
validity of this covenant nor to impose any liability
whatsoever upon the County of Suffolk or any officer or
employee thereof.
These covenants and restrictions shall run with the land and
shall be binding upon the DECLARANT, its successors and
assigns, and upon all persons or entities claiminq under
them, and may be terminated, revoked or amended only with
the written consent of the DECLARANT.
The declarations set forth in the WHEREAS clauses contained
herein shall be deemed and construed to be promises,
covenants and restrictions as if fully repeated and set
forth herein.
If any section, subsection, paragraph, clause, phrase or
provision of these covenants and restrictions shall, by a
Court of competent jurisdiction, be adjudged illegal,
unlawful, invalid, or held to be unconstitutional, the same
shall not affect the validity of these covenants as a whole,
or any other part or provision hereof other than the part so
adjudged to be illegal, unlawful, invalid,' or
unconstitutional.

Local Law #32-1980 - The DECLARANT represents and warrants
that he has not offered or given any gratuity to any
official, employee, or agent of Suffolk County, New York
State or of any political party, with the purpose of intent
of securing favorable treatment with respect to the
performance of an agreement, and that such person has read

,We reside in the proximity of Laurel Lake in Mattituek. New York and we are
disturbed about the unfulfilled promises that the recent administration had proclaimed
in their pre-election platform. That is. Suffolk County's sales tax was increased to
8~}% so that ~$ could be used to acquire environmentally sensitive lands through the
Revenue Sharing Element of the Suffolk County Drinking Water Protection Program.
It's our understanding that the purpose of this open space program is to purchase large
tracts of land to preserve the aequifer water quality by preventing further development
and residential subdivision.
By Resolutions of June 4. 1991 and August 11, 1992, the Southold Town Board
requested that six parcels of land located around Laurel Lake be added to the County's
acquisition list. Of the six purchases requested, only one has been consummated as
of this date. Laurel Lake is one of natures's wonders and its pristine quality is still
intact, We are paying the increased sales tax based on the prior administration's pre-
election promise and we ask ourselves WHY? How come more ]and been hasn't been
acquired? Certainly, the reason for this inactivity is not a lack of funds. The county
recently reneged on their promise to purchase Robin's Island for $9 million. The $9
mi{lion was already allocated for this open space program purchase and Robins Island
is located in the Town of Southold - where is this money being spent now?
Laurel Lake is planned to be used as a future reservoir for the Town of Southold
and possibly for the Town of Riverhead as well. The five parcels remaining on
Southold's request list comprise approximately 200 acres of vacant land, mostly farmland.
We do not want to see this land become developed with major residential subdivisions.
Development of this type will surely destroy our future water supply and the pristine
quality of Laurel Lake. We want action - if you can not or will not spend the money
which was already allocated for the b~[vik C~ounty Drinking Water Protection Program,
then repeal this tax~

reviewed by Planning Staff and the Town Attorney.
Ail of the Planning Board's requirements have been included in
the document. Please add the following paragraph to the document
and then record it in the office of the County Clerk:
These covenants and restrictions can be modified only at
the request of the then owner of the premises with the
approval of a majority plus one of the Planning Board of
the Town of Southold after a public hearing. Adjoining
property owners shall be entitled to notice of such public
hearing but their consent to such modification shall not be
required.
A copy of the recorded document must be submitted to this office
and the liber and page number must be noted on the final maps.

approval on the surveys dated February 23, 1994 and authorize the Chairman
to endorse the final surveys subject to fulfillment of the following condition.
This condition must be met within six (6) months of the date of this resolution.
1. The filing of new deeds pertaining to the merger of the 9,300 square feet

At this point, is anybody here that would like to address the Board on this
application. Would you please state your name for the record?
Barbara Sayer: I'm a neighbor. I live on Laurel Lake Road (inaudible). What I
want to give you now is a letter that the Laurel Lake Property Owners
Association wrote to Mr. Gaffney, the County Executive, and it mentions
Southold Town Board and six parcels and the quarter percent sales tax, and so
forth, and I just wanted to submit this for your record on this matter.
Mr. Ward.: Alright, thank you very much. Is there anybody else that would Pike
to address the Board on this particular application? If not, I think we're all in
order to close the hearing?

referred to as the Declarant, slates as ~ollows:
WHEREAS, Declarant is the owner of certain real property
situate in the Town of Southold, Suffolk County, New York, more
particularly bounded and described as set forth in Schedule
annexed hereto as provided by the Declarant; and
WHEREAS, for and in consideration of the granting of a
subdivision application entitled, 'Minor Subdivision Plat for
Peconic Homes, Inc.' and by Resolution dated October 5, 1993, a
Copy of which is attached hereto and made a part hereof, and as a
condition of granting said approval, the Town of Southold Planning
Board has required that the within Declaration be recorded in the

The following resolutions were adopted by the Southold Town
Planning Board at a meeting held on Monday, October 4, 1993:
WHEREAS, a major sub~ivision application ~or the Peconic Homes
property has been before the Planning Board since 1988 and has
no= proceeded due to the applicant's decision to not proceed i~
response to the SEORA Positive Declaration issued by the
Planning Board on June 7, 1989; an~
WHEREAS, the applicant has now applied for a minor subdivision
to create a building lot with an existing house in order to
separate the use from the overall property which is the subject
of the previously mentioned major subdivision; and
WHEREAS, the Plaru%ing Board currently has before it two
different applications for the above mentioned parcel; one for a
major subdivision, and the other for a minor subdivision; and
wHEREAS, the Board can not proceed simultaneously with two
different applications for the same parcel, its review of ~he
major sub~ivision will be put on hold; and
WHERF~, the intent of the minor subdivision application is to
separate the existing dwelling (shown as proposed Lot Number 2
on the minor su~division map dated July 30, 1993) from =he total
'45.97 acre parcel; and
WHEREAS, Lot Number i (the remaining 43.5784 acre parcel shown
on the minor subdivision map dated July 30, 1993), which

currently contains a mobilu home and numerous greenhouses~ shall
not be improved with any additional dwelling units unless
certain conditions are met~ and
Be it therefore RESOLVED that the Southold Town Plan~ing Board
grant sketch approval on the map dated July 30, 1993 subject to
the following condition:
1. A Declaration of covenants and Restrictions including
the following terms must be recorded prior to any
approval of the subdivision:
A. Prior to the issuance of any building permit for
any dwellin~ unit on proposed Lot Number 1, such
application shall be reviewed by the Planning
Board. The Planning Board shall determine whether
the additional impact warrants inclusion of the
action within the June 7, 1989 Positive Declaration
or requires any further environmental review.
B. Prior to the issuance of any building permit for
any dwelling unit on proposed Lot Number 1, such
application shall be reviewed b y the Planning
Board. The Planning Board shall determine what road
improvements are necessary due to the impact of the
additional dwelling ~nit.
C. A park and playground fee will be required at such
time that a dwellin~ unit is allo~ed on Lot #1.
Be it further RESOLVED that the Southold Town Planning Board
start the lead agency coordination process on this Type 1
action. This action is classified as a Type 1 action since it is
located within a Special Groundwater Protection Area which is a
designated Critical Environmental Area.
Sketch plan approval is conditional upon submission of final
maps within six months of the date of sketch approval, unless an
'extension of time is requested by the applicant, and granted by
the Planning Board. The final maps (2 mylars and 5 paper
prints} must contain a current stamp of Health Department
approval, and must be submitted before a final public hearing
will be set.
The sketch plans have been referred to the Suffolk County
Planning Commission for review. You will be,noti£ied upon
receipt of the report from the Co~ission. The Planning Board
may require additional covenants and restrictions upon review of
the report. Therefore, it is not necessary to prepare the draft
Declaration o£ Covenants and Restrictions at this time.
The sketch plans have also been referred to the Mattituck Fire
District for their recon~enda=ion as to whether a firewell is
necessary for fire protection.

DECLARATION OF COVENANTS AND RESTRICTIONS
THIS DECLARATION made by PECONIC HOMES CORPORATION this

located at (no ~) Sound Ave., Mattituck, New York 11952 hereinafter
referred to as the DECLARANT, as the owner of premises described in
Schedule "A" annexed hereto (hereinafter referred to as the
PREMISES) desires to restrict the use and enjoyment of said PREMISES
and has for such purposes and determined to impose on said PREMISES
covenants and restrictions and does hereby declare that said
PREMISES shall be held and shall be conveyed subject to the
following covenants and restrictions:
WHEREAS, DECLARANT has made application to the Suffolk
County Department of Health Services (hereinafter referred
to as the DEPARTMENT) for a permit to construct and/or
approval of plans for a single family residence, a
subdivision or development or other construction project on
the PREMISES; and

site private well; and
WHEREAS, the test wells sampled for the PREMISES indicated a
groundwater supply that had contamination in excess of the
minimum drinking water standard and/or guidelines of the
State of New York and contained excess of the following:

A complete list of analysis results is attached hereto as
Schedule C, and each chemical parameter which exceeds 60% of
the acceptable level is.indicated with an asterisk; and
WHEREAS, the County of Suffolk Department of Health Services
has agreed to issue a permit only if there be a record
covenant that the necessary water conditioning equipment be
installed so as to meet the quality standards for drinking
water; it is DECLARED and COVENANTED by DECLARANTS, their
heirs or successors and assigns forever, that no residence
upon the above described property will be occupied prior to
the installation of the necessary water conditioning
equipment so that the water, when conditioned, meets the
said minimum quality standards for drinking water of the
State of New York and evidence of the same is furnished to
the Suffolk county Department of Health Services for their
written approval.

The DECLARANT, its successors and/or assigns shall set forth
these covenants, agreements and declarations in any and al
leases to occupants, tenants and/or lessees of the above
described property and shall, by their terms, subject same
to the covenants and restrictions contained herein. Failure
of the DECLARANT, its successors and/or assigns to so
condition the leases shall not invalidate their automatic
subjugation to the covenants and restrictions.
Ail of the covenants and restrictions contained herein shall
be construed to be in addition to and not in derogation or
limitation upon any provisions of local, state and federal
laws, ordinances, and/or regulations in effect at the time
of execution of this agreement, or at the time such laws,
ordinances, and/or regulations may thereafter be revised,
amended or promulgated.
This document is made subject to the provisions of all laws
required by law or by their provisions to be incorporated
herein and they are deemed to be incorporated herein and
made a part hereof, as though fully set forth.
The aforementioned Restrictive Covenants shall be
enforceable by the County of Suffolk, State of New York, by
injunctive relief or by any other remedy in equity or at

letter dated October 5, 1993, concerning the above-referenced project. The Depaxtment has no
objection to your designation as lead agency.
Based on a review of the subject coordination, the Depar-~ent offers the following comments.
However, the Department wishes to reserve its right to provide more detailed information within the
comment period(s) established for this action. Also, these comments should not be construed as an
implicit SCDHS approval or rejection of the project. All applications are reviewed thoroughly with
respect to Suffolk County Sanitary Code (SCSC) concerns by appropriate depam'nantal personnel
when SCDHS applications are completed.
The above described parcel of land has been reviewed in the past by the Office of Water and
Wastewater Management for realty subdivision with different configurations. The current proposal
is pending with Wastewater Management.
The applicant must comply with the requirements of the Suffolk County Sanitary Code and
relevant construction standards for water supply and sewage disposal. Design and flow
specifications, subsurface soil conditions, and complete site plan details are essential to the review
of this project. These considerations are reviewed completely at the time of SCDHS application.

FULL ENVIRONMENTAL ASSESSMENT FORM
Purpose: The full [!Al: is designed to help applicants and agencies determine, in an orderly manner, whether a proiect
or action may be significant. The question of whether an action may be significant is not always easy to answer, Frequent-
ly, there are aspects of a project that are subjective or unmeasureable. It is also understoorJ that those who determine
significance may have little or no formal knowledge of the environment or may he technically expert in environmental
analysis. In addition, many who have knowledge in one particular area may not be aware of tile broader concerns affecting
the question of significance.
The full FAF is intended to provide a method whereby applicants and agencies can be assured that the determination

The following resolutions were adopted by the Southold Town
Planning Board at a meeting held on Monday, October 4, 1993:
WHEREAS, a major subdivision application for the Peconic Homes
property has been before the Planning Board since 1988 and has
not proceeded due to the applicant's decision to not proceed in
response to the SEQRA Positive Declaration issued by the
Planning Board on June 7, 1989; and
WHEREAS, the applicant has now applied for a minor subdivision
to create a building lot with an existing house in order to
separate the use from the overall property which is the subject
of the previously mentioned major subdivision; and
WHEREAS, the Planning Board currently has before it two
different applications for the above mentioned parcel; one for a
major subdivision, and the other for a minor subdivision; and
WHEREAS, the Board can not proceed simultaneously with two
different applications for the same parcel, its review of the
major subdivision will be put on hold; and
WHEREAS, the intent of the minor subdivision application is to
separate the existing dwelling (shown as proposed Lot Number 2
on the minor subdivision map dated July 30, 1993) from the total
45.97 acre parcel; and
WHEREAS, Lot Number 1 (the remaining 43.5784 acre parcel shown
on the minor subdivision map dated July 30, 1993), which

maps within six months of the date of sketch approval, unless an
extension of time is requested by the applicant, and granted by
the Planning Board. The final maps (2 mylars and 5 paper
prints) must contain a current stamp of Health Department
approval, and must be submitted before a final public hearing
will be set.
The sketch plans have been referred to the Suffolk County
Planning Commission for review. You will be notified upon
receipt of the report from the Commission. The Planning Board
may require additional covenants and restrictions upon review of
the report. Therefore, it is not necessary to prepare thc draft
Declaration of Covenants and Restrictions at this time.
The sketch plans have also been referred to the Mattituck Fize
District for their recommendation as to whether a firewell is
necessary for fire protection.
