The applicant had previously received approval of use of this

Thornton E. Smith. At the time the application was made, John
Simicich, Jr., advised the board that he intended to renovate the
house located on the parcel which is the subject of this set-off

subdivision at Mattituck
Dear Mr. Smith:
The Planning Board inspected the alternate road proposal,
which Mr. Young had staked, on March 13, 1987.
It was the consensuS of the Board that the line of site
may be acceptable for this location.
The Planning Board requests that your surveyor prepare
a survey with'this road location and the new right-of-waY
on Sound Avenue which the State is proposing to determine
how this would line up. The Board also requestS that you
have a survey prepared indicating a connecting road to the
road proposed within the subdivision of Mohring as indicated
in your correspondence of March 13, to see how this would
line up.
with regard to the corner lot on Bergen Avenue (to be
conveyed to John simicich), the Board requests that there be
covenants and restrictions, that there will be no residential
strutures on this property.
If you have any questions with regard to this, please
don't hesitate to contact our office.

RE:
Proposal for
John Simicich, Jr.
SCTM# 1000-121-1-p/ol
Dear Mr. Cuddy:
To confirm the conversation Melissa Spiro had with your
office, the following items are necessary for the above
mentioned proposal~
1. The fee paid to the Planning Board was for a set off.
The parcel in question was the subject of a previous set
off, thus the proposal now set forth must proceed as a
minor subdivision. Please adjust the fee accordingly.
2. The Questionnaire indicates that there are wetlands on
the parcel. Please have the wetlands flagged and marked on
the map.
3. There will have to be a Restriction placed on Lot 1
similar to that which was placed on the previous set off.
The restriction previously stated that "there should be no
more than two residential dwellings together with a winery
complex .... "The restriction for the proposed Lot 1 should
change the two dwellings to one. The proposal has been
sent to the Suffolk County Planning Commission for their
review. It is suggested that the Declaration of Covenants
and Restrictions be prepared after the review of the
Commission, as they are likely to ask for additional
restrictions.
Upon receipt of maps showing the wetland, the Planning
Board will begin theLead Agency coordination process.
Please contact the office if ~ou have any questions
regarding the above.

Re: Set-Off (John Simicich, Jr.)
Dear Mr. Orlowski:
With regard to the above set-off, enclosed please find the
following:
1. Application for Approval of Plat;
2. Short Environmental Assessment Form;
3. Questionnaire Form;
4. Fee in the amount of $250.00 and
5. Twelve (12) survey maps.
The applicant had previously received approval of use of this
entire parcel in connection with an exchange of a like parcel with
Thornton E. Smith. At the time the application was made, John
Simicich, Jr., advised the board that he intended to renovate the
house located on the parcel which is the subject of this set-off
application so that it may be used for his daughter. Mr.
Simicich, in connection with transferring the property to his
daughter and son-in-law and obtaining a mortgage for renovation,
requires that the parcel be set-off at this time.
We request that you advise us when this matter will appear on the
agenda so that we may appear before the board.
