THIS IS TO CERTIFY THAT THE FOLLOWING RESOLUTION WAS ADOPTED BY THE
SOUTHOLD TOWN BOARD AT A REGULAR MEETING HELD ON JULY 28, 1992:
WHEREAS, the Suffolk County Water Authority has requested a waiver of
the Excavation Permit process with respect to the premises at 365 Dogwood
Lane, Mattituck, New York; and
WHEREAS, the Suffolk County Water Authority agrees to abide by the rules
and regulations set forth in Chapter 83, Street Excavations, of the Code
of the Town of Southold, and perform all work under the supervision of
Superintendent of Highways Jacobs; now, therefore, be it
RESOLVED that the Town Board of the Town of Southold hereby grants
permission to the Town Clerk to issue an Excavation Permit Waiver to the
Suffolk County Water Authority with .respect to the premises at 365 Dog-
wood Lane, Mattituck, all in accordance with the approval of Superintendent
of Highways Jacobs.

Gentlemen:
The owner of the above captioned residence has requested and paid all re-
quired fees to the Suffolk County Water Authority to install a 3/4" water service.
The Authority requests a waiver of all bond and permit fees, so that we may
install this service.
All work will be done under the supervision of the Southold Town Highway
Superintendent.
