State Environmental Quality Review
FULL ENVIRONMENTAL ASSESSMENT FORM
Purpose: The full EAF is designed to help applicants and agencies determine, in an orderly manner, whether a project
or action may be significant. The question of whether an action may be significant is not always easy to answer. Frequent-
ly, there are aspects of a project that are subjective or unmeasureable. It is also understood that those who determine
significance may have little or no formal knowledge of the environment or may be technically expert in environmental
analysis. In addition, many who have knowledge in one particular area may not be aware of the broader concerns affecting
the question of significance.
The full EAF is intended to provide a method whereby applicants and agencies can be assured that the determination
process has been orderly, comprehensive in nature, yet flexible to allow introduction of information to fit a project or action.
Full EAF Components: The full EAF is comprised of three parts:
Part 1: Provides objective data and information about a given project and its site. By identifying basic project
data, it assists a reviewer in the analysis that takes place in Parts 2 and 3.
Part 2: Focuses on identifying the range of possible impacts that may occur from a project or action. It provides
guidance as to whether an impact is likely to be considered small to moderate or whether it is a potentially-
large impact. The form also identifies whether an impact can be mitigated or reduced.
Part 3: If any impact in Part 2 is identified as potentially-large, then Part 3 is used to evaluate whether or not the
impact is actually important.

• In completing the form the reviewer should be guided by the question: Have my responses and determinations been
reasonable? The reviewer is not expected to be an expert environmental analyst.
• Identifying that an impact will be potentially large (column 2) does not mean that it is also necessarily significant.
Any large impact must be evaluated in PART 3 to determine significance. Identifying an impact in column 2 simply
asks that it be looked at further.
• The Examples provided are to assist the reviewer by showing types of impacts and wherever possible the threshold of
magnitude that would trigger a response in column 2. The examples are generally applicable throughout the State and
for most situations. But, for any specific project or site other examples and/or lower thresholds may be appropriate
for a Potential Large Impact response, thus requiring evaluation in Part 3.
• The impacts of each project, on each site, in each locality, will vary. Therefore, the examples are illustrative and
have been offered as guidance. They do not constitute an exhaustive list of impacts and thresholds to answer each question.

the most appropriate location for access to the lot is over the existing

Dear Melissa,
I have submitted to your board the additional $900.00 check as per your
request, along with the letter of authorization.
I have made note of the tax map change with the surveyor and the maps will be
changed to reflect the same.
As to the second page of your letter:
1. The partnership feels that they would like to leave the 5 acres free of any
restrictions with the exception of number five which refers to the 75' wide buffer
area.
2. With reference to number two, the use of the Plock ROW does not eliminate
coming within the 75' wetland area. Furthermore, the proposal of the 20' ROW
where it is indicated is to create an estate lot with greater value because of the
access.
3. With respect to number three, there are many partners in the particular parcel
of land and it has been difficult to reach an agreement between all of the parties.
Therefore it has been agreed to handle each property separately.
4. With reference to number four. The use of this parcel not yet known.

Dear Ms. Geraghty:
I have reviewed the application materials for the above mentioned
subdivision proposal. The following items must be submitted in order to
complete the application:
1. As we discussed, the proposal is for a 2 lot minor subdivision which
requires an application fee of $1000.00 plus $500.00 per lot. A
total application fee of $2000.00 is required for this proposal. The
application fee submitted was in the amount of $1500.00. Therefore
$500.00 is outstanding.
2. You must submit authorization from all owners of the property for
you to proceed with the proposed subdivision.
3. The proposed map shows the tax map number of the subject property
as 79-5-20.4. The Town's property records show the subject
property as SCTM# 1000-79-5-20.7. The tax map number must be
corrected. Please note that it is not necessary to make this
correction now. However, it must be done with the next submission
of the map.
Until receipt of items 1 and 2 above, the application is considered
incomplete.
The Planning Board reviewed the proposed subdivision at the June 2,
1997 work session. The Planning Board's comments are listed below:

1. When you originally submitted the concept plan to the Planning
Board in November 1996, you mentioned that the applicant would be
placing a restriction on the 5 acre parcel for no further
subdivision. This is not mentioned in the current application. Please
clarify.
2. The 20' wide right-of-way which is shown as access to proposed Lot
1 is located within 75' of the wetland areas shown on the map. The
Planning Board feels that the most appropriate location for access to
proposed Lot 1 is over the existing road which provides access to
the lots on the Plock property. The Planning Board would like you
to explore this option. The Board is willing to support you in any
efforts to gain access over the Plock road.
3. The Planning Board feels that a plan including the adjacent property
to the east would allow for a better design of the waterfront by
allowing for wider waterfront lots. The Planning Board would like
you to explore this option.
4. The Planning Board has questioned what your client's plans are for
proposed Lot 2.
5. The Planning Board required a 75' wide buffer area adjacent to the
bay as part of the Plock subdivision. The Planning Board is
requiring the same for the Kerester subdivision. The buffer area
must be shown on the map.
Please respond to the above mentioned comments. In addition, please note
that upon receipt of a complete application, the Planning Board will be
initiating the environmental review. A $400.00 environmental fee is
required.

toward the payment of the mortgage debt, notwithstanding the fact that the amount owing thereon may not
then be due and payable; and the said mortgagor hereby agrees, upon request, to make, execute and deliver
any and all assignments and other instruments sufficient for the purpose of assigning said awards to the mort-
gagee, free, clear and discharged of any encumbrances of any kind or nature whatsoever.
AND the mortgagor covenants with the mortgagee as follows:
1. That the mortgagor will pay the indebtedness as hereinbefore provided.
2. That the mortgagor will keep the buildings on the premises insured against loss by fire for the benefit
of the mortgagee; that he will assign and deliver the policies to the mortgagee; and that he will reimburse the
mortgagee for any premiums paid for insurance made by the mortgagee on the mortgagor's default in so insur-
ing the buildings or in so assigning and delivering the policies.
3. That no building on the premises shall be altered, removed or demolished without the consent of the
mortgagee.
4. That the whole of said principal sum and interest shall become due at the option of the mortgagee: after
default in the payment of any instalment of principal or of intetest for fifteen days; or after default in the pay-
ment of any tax, water rate, sewer rent or assessment for thirty days after notice and demand; or after default
after notice and demand either in assigning and delivering the policies insuring the buildings against loss
by fire or in reimbursing the mortgagee for premiums paid on such insurance, as hereinbefore provided; or
after default upon request in furnishing a statement of the amount due on the mortgage and whether any offsets
or defenses exist against the mortgage debt, as hereinafter provided. An assessment which has been made
payable in instalments at the application of the mortgagor or lessee of the premises shall nevertheless, for the
purpose of this paragraph, be deemed due and payable in its entirety on the day the first instalment becomes
due or payable or a lien.
S. That the holder of this mortgage, in any action to foreclose it, shall be entitled to the appointment of a
receiver.
6. That the mortgagor will pay all taxes, assessments, sewer rents or water rates, and in default thereof,
the mortgagee may fay the same.
7. That the mortgagor within five days upon request in person or within ten days upon request by mail
will furnish a written statement duly acknowledged of the amount due on this mortgage and whether any offsets
or defenses exist against the mortgage debt.
8. That notice and demand or request may be in writing and may be served in person or by mail.
9. That the mortgagor warrants the title to the premises.
10. That the fire insurancepolicies required by paragraph No. 2 above shall contain the usual extended cover-
age endorsement; that in addition thereto the mortgagor, within thirty days after notice and demand, will keep
the premises insured against war risk and any other hazard that may reasonably be required by the mortgagee.
All of the provisions of paragraphs No. 2 and No. 4 above relating to fire insurance and the provisions of
Section 254 of the Real Property Law construing the same shall apply to the additional insurance required by
this paragraph.
11. That in case of a foreclosure sale, said premises, or so much thereof as may be affected by this mortgage,
may be sold in one parcel.
12. That if any action or proceeding be commenced (except an action to foreclose this mortgage or to collect
the debt secured thereby),to which action or proceeding the mortgagee is made a party, or in which it becomes
necessary to defend or uphold the lien of this mortgage, all sums paid by the mortgagee for the expense of
any litigation to prosecute or defend the rights and lien created by this mortgage (including reasonable counsel
