The following action was taken by the Southold Town Board of Trustees at their Regular
Meeting held on January 21,2004:
RESOLVED, that the Southold Town Board of Trustees APPROVE the Amendment to Permit
#5338 to add to and include in the project the inkindlinplace replacement of an additional 142+/-
linear ft. of existing bulkhead, with the condition a 10' non-turf buffer is installed landward of
the bulkhead and as depicted on the plan prepared by Sea Level Mapping and James E.
Fitzgerald, Jr., last dated November 21,2003.
This is not a determination from any other agency.

At the meeting of the Southold Town Conservation Advisory Council held Tuesday,
January 13, 2004, the following recommendation was made:

A CONDITION of the Amendment application of FAYE REYNOLDS to Amend Permit

Dear Mr. Fitzgerald:
The following action was taken by the Southold Town Board of Trustees at their Regular
Meeting held on Wednesday, April 30, 2003:
RESOLVED that the Southold Town Board of Trustees grants a One-Year Extension to WetIand
Permit #5338. Permit is to remove and replace in the same location 101 '+/- of existing retaining
wall and 70'+/- of existing decking between the bulkhead and the retaining wall. Remove any
fill that enters the waterway and replace behind the bulkhead or retaining wall. Fill as necessary
behind the bulkhead and retaining wall to maintain established grades using an estimated 20 cy.
of spoil from an associated separately permitted dredging activity. Install 6'X 20' floating dock,
secured by two piles, and a 4'X 16' access ramp. Install wood jetty 12' seaward and 6' landward
of existing jetty at eastern end ofproject. Install a 10' non-turf buffer landward of the bulkhead.
This is not an approval from any other agency.

Dear Mr. Fitzgerald, Jr.
The following action was taken by the Board of Town Trustees during a Regular Meeting, held
on May 24, 2001, regarding the above matter.
WHEREAS, PROPER-T PERMIT SERVICES. on behalf of EDWARD & FAYE
REYNOLDS applied to the Southold Town Trustees for a permit under the provisions ofthe
Wetland Ordinance of the Town of South old, application dated May 3,2001,
WHEREAS, said application was referred to the Southold Town Conservation Advisory Council
for their findings and recommendations, and
WHEREAS, a Public Hearing was held by the Town Trustees with respect to said application on
May 24, 2001 at which time all interested persons were given an opportunity to be heard, and,
WHEREAS, the Board members have personally viewed and are familiar with the premises in
question and the surrounding area, and,
WHEREAS, the Board has considered all the testimony and documentation submitted
concerning this application, and,
WHEREAS, the structure complies with the standard set forth in Chapter 97-18 of the Southold
Town Code. . /. .

WHEREAS, the Board has determined that the project as proposed will not affect the health,
safety and general welfare ofthe people of the town,
NOW THEREFORE BE IT.
RESOLVED, that the Board of Trustees approved the application of Proper-T Permit Services
on behalf of EDWARD & FAYE REYNOLDS for a Wetland Permit to remove and replace in
the same location 101' +/ - of existing retaining wall, remove and replace in the same location
70' +/- of existing decking, between the bulkhead and the retaining wall. Remove any fill
material that enters the waterway and replace it behind the bulkhead or retaining wall. Fill as
necessary behind the bulkhead and retaining wall to maintain established grades using an
estimated 20 cy of spoil from an associated separately permitted dredging activity. Insta1l6'x20'
floating dock, secured by two piles, an 4'xI6' access ramp. Install wood jetty 12' seaward and
6' landward of existing jetty at eastern end of project. Providing that the boats are placed on
either side the float does not protrude further than the existing tie off poles (and a new sketch
showing that) a ten foot non-turf buffer where all the bulkhead is replaced.
BE IT FURTHER RESOLVED that this determination should not be considered a determination
made for any other Department or Agency which may also have an application pending for the
same or similar project.
Permit to construct project will expire two years from the date it is signed. Fees must be paid, if
applicable, and permit issued within six months ofthe date ofthis notification.

Two inspections are required and the Trustees are to be notified when project is started and on
completion of said project.

identified by Suffolk County Tax Map No. 1000-144-5-16, and that all work will be done in
the manner set forth in the present application and as may be approved by the Southold Board
of Town Trustees. We agree to hold the Town of South old and the Town Trustees harmless
and free from any and all damages and claims arising under or by virtue of said permit(s), if
granted. We hereby authorize the Town Trustees or their agent(s) or representative(s) to enter
upon our property to inspect the premises in conjunction with the review of the present
application.
