I am requesting a final inspection by the Board of Southold Town Trustees for 770
Old Salt Road, Mattituck NY Permit Number 6071. (copy of permit 6071 enclosed)
It is my understanding that although the inspection fee was paid for the final
inspection in 2005 the inspection was never performed..
I give my permission to the Town Trustees to access the property in order to inspect
the work done per permit# 6071 so that a certificate of compliance can be issued.

Dear Mr. Herrmann:
The Board of Town Trustees took the following action during its regular meeting held on
Wed., January 19, 2005 regarding the above matter:
WHEREAS, En-Consultants, Inc. on behalf of JOAN O'LEARY applied to the Southold
Town Trustees for a permit under the provisions of Chapter 97 of the Southold Town
Code, the Wetland Ordinance of the Town of Southold, application dated December 29,
2004 and,
WHEREAS, said application was referred to the Southold Town Conservation Advisory
Council for their findings and recommendations, and,
WHEREAS, a Public Hearing was held by the Town Trustees with respect to said
application on January 19, 2005, at which time all interested persons were given an
opportunity to be heard, and,
WHEREAS, the Board members have personally viewed and are familiar with the
premises in question and the surrounding area, and,
WHEREAS, the Board has considered all the testimony and documentation submitted
concerning this application, and,
WHEREAS, the structure complies with the standards set forth in Chapter 97 of the
Southold Town Code,

WHEREAS, the Board has determined that the project as proposed will not affect the
health, safety and general welfare of the people of the town,
RESOLVED, that the Board of Trustees approve the application of JOAN O'LEARY to
replace (within 18") approx. 100 linear ft. of existing concrete seawall with fiberglass
bulkhead and backfill with approx. 75 cy clean sand fill to be trucked in from an upland
source. For bulkhead installation purposes, remove and replace 3' x 8' platform; with the
condition of a 12' non-turf buffer and all as depicted on the plan drawn by En-
Consultants Inc. dated 12/20/04.
Permit to construct and complete project will expire two years from the date the permit
is signed. Fees must be paid, if applicable, and permit issued within six months of the
date of this notification.
Inspections are required at a fee of$50.00 per inspection. (See attached schedule.)
This is not a determination from any other agency.

A meeting of the Southold Town Conservation Advisory Council was held Tues.,
January 11, 2005. The following recommendations were made:
DISCUSSION AND RECOMMENDATION OF WETLAND PERMIT & COASTAL
EROSION PERMIT APPLICATIONS
Moved by Nicholas Dickerson, seconded by Don Wilder, it was
RESOLVED to recommend to the Southold Town Board of Trustees APPROVAL WITH
A CONDITION of the Wetland Permit application of JOAN O'LEARY to replace (within
18") approx. 100 linear ft. of existing concrete seawall with fiberglass bulkhead and
backfill with approx. 75 cy. clean sand fill to be trucked in from an upland source. For
bulkhead installation purposes, remove and replace TX 8' platform.
Located: 770 Old Salt Rd., Mattituck. SCTM#144-5-14
The CAC recommends Approval of the application with the Condition the seawall is
replaced in-place and the existing 12' buffer is maintained.
