Dear Ms. Lizewski:
The Board of Town Trustees took the following action during its regular meeting held on
Wednesday January 19, 2005 regarding the above matter:
WHEREAS JUDIE LIZEWSKI applied to the Southold Town Trustees for a permit under
the previsions of Chapter 97 of the $outhold Town Code, the Wetland Ordinance of the
Town of Southold, application dated December 21, 2004, and
WHEREAS, said application was referred to the Southold Town Conservation Advisory
Council for their findings and recommendations, and,
WHEREAS, a Public Hearing was held by the Town Trustees with respect to said
application on January 19, 2005 at which time all interested persons were given an
opportunity to be heard, and,
WHEREAS, the Board members have personally viewed and are familiar with the
premises in question and the surrounding area, and,
WHEREAS, the Board has considered all the testimony and documentation submitted
concerning this application, and,
WHEREAS, the structure complies with the standards set forth in Chapter 97 of the
Southold Town Code,
WHEREAS, the Board has determined that the project as proposed will not affect the
health, safety and general welfare of the people of the town,
RESOLVED, that the Board of Trustees approve the application of JUDIE LIZEWSKI to
construct a single-family dwelling with on-site sewage disposal system and construct a
4' x 26' fixed walkway dock, with the conditions of a 50' non disturbance buffer, hay
bales, and gutters and drywells on the house and all as depicted on the plan surveyed
by Joseph Ingegno last dated April 29, 2002.
Permit to construct and complete project will expire two years from the date the permit
is signed. Fees must be paid, if applicable, and permit issued within six months of the
date of this notification.
Inspections are required at a fee of $50.00 per inspection. (See attached schedule.)
This is not a determination from any other agency.

RESOLVED to recommend to the Southold Town Board of Trustees APPROVAL WITH
CONDITIONS of the Wetland Permit application JUDIE LIZEWSKI to construct a
single-family dwelling with on-site sewage disposal system and private well, and
construct a 4'× 26' fixed walkway dock.

The CAC recommends Approval of the application with the Condition a landscape plan
is submitted specifying what trees are to be removed and what trees are to remain,
before the permit is issued, and a 50' non-disturbance buffer in order to protect water
quality.

private well - house plans in line with the homes to the east - the house will not extend no
further seaward - construct a 4'x26' fixed walkway dock - 50 foot non-disturbance buffer.
BE IT FURTHER RESOLVED that this determination should not be considered a determination
made for any other Department or Agency, which may also have an application pending for the
same or similar project.
Fee must be paid, if applicable and permit issued within six months of the date of this
notification.

The CAC recommends Disapproval of the application because the proposed house and
dock were not staked. The CAC recommends that the proposed drywells and access
path to the dock be staked and a cross-section submitted for the dock.
