This application must be filled in by typewriter or ink and submitted to the Building Department with the following:
A. For new building or new use:
I. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or
topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal (S-9 fonn).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains less than 2/1 0 of I % lead.
5. Commercial building, industrial building, multiple residences and similar buildings and installations, a certificate
of Code Compliance from architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
B. For existing buildings (prior to April 9, 1957) non-conforming nses, or bnildings and "pre-existing" land uses:
I. Accurate survey of property showing all property lines, streets, building and unusual natural or topographic
features.
2. A properly completed application and consent to inspect signed by the applicant. If a Certificate of Occupancy is
denied, the Building Inspector shall state the reasons therefor in writing to the applicant.

Dear Mr. Lee:
At your written request, date received on April 18, 2006, for a Certificate of Occupancy final site
inspection, Planning Department staff performed a site visit on April 24, 2006.
In reference to the above, the Planning Board held a work session on April 24, 2006 to review the
site plan dated July 26, 2004, approved December 14, 2004 and visit staff comments. The Planning
Board accepts the as built changes as described by staff, including the absence of six (6) ISO-watt
spotlights and gravel parking area around the new agricultural building. This is to inform you that
the site plan requirements have been satisfied conditional of the following item:
1. All outdoor lighting will require review and approval from the Planning Board. Outdoor
lighting shall be shielded so that the light source is not visible from adjacent properties and
roadways. Lighting fixtures shall focus and direct the light in such a manner as to contain the
light.and glare within property boundaries. The lighting must meet the Town Code
reqUIrements.
This letter does not condone any additional changes from the approved site plan or approvals from
other agencies, if required.

Dear Sirs:
Please find the enclosed truss design for the truss that was used for this job.
I am also writing to confirm and advise that the footings and foundation were
observed to have been installed in accordance to my plans dated 04-22-04
including the following:
1.) Footings were placed 4'9" below grade and were drilled the
diameter specified on the plans.
2.) 4-46M Base Anchors were nailed near the bottom of each column
with 3-0.177" x 2 y,' stainless nails in each anchor.
3.) A 9" thick red i-mix footing was poured and an additional 8" of redi-
mix was placed around the columns when setting.

INSTRUCTIONS
a. This application MUST be completely filled in by typewriter or in ink and submitted to the Building Inspector with 3
sets of plans, accurate plot plan to scale. Fee according to schedule.
b. Plot plan showing location of lot and of buildings on premises, relationship to adjoining premises or public streets or
areas, and waterways.
c. The work covered by this application may not be commenced before issuance of Building Permit.
d. Upon approval ofthis application, the Building Inspector will issue a Building Permit to the applicant. Such a permit
shall be kept on the premises available for inspection throughout the work.
e. No building shall be occupied or used in whole or in part for any purpose what so ever until the Building Inspector
issues a Certificate of Occupancy.
f. Every building permit shall expire if the work authorized has not commenced within 12 months after the date of
issuance or has not been completed within 18 months from such date. If no zoning amendments or other regulations affecting the
propert':.have been enacted in the interim, the Buildi.ng Inspector may authorize, in writing, the extension of the permit for an
addition six months. Thereafter, a new permit shall be required.
APPLICATION IS HEREBY MADE to the Building Department for the issuance of a Building Permit pursuant to the
Building Zone Ordinance of the Town of Southold, Suffolk County, New York, and other applicable Laws, Ordinances or
Regulations, for the construction of buildings, additions, or alterations or for removal or demolition as herein described. The
applicant agrees to comply with all applicable laws, ordinances, building code, housing code, and re ations, and to admit
authorized inspectors on premises and in building for necessary inspections.

Dear Mr. Kelly:
The following resolution was adopted by the Southold Town Planning Board at a
meeting held on Monday, December 13, 2004:
The final public hearing was closed.
WHEREAS the applicant William Kelly proposes construction of a 5,640 sq. ft.
agricultural storage building on a 14.36 acre parcel in the A-C Zone; and
WHEREAS Fred and Karen Lee are the owners of the property known as SCTM#1000-
84-5-1.2 & 1.3 and the property is located at 25180 CR 48, approximately 781' elo
Bridge Lane, on the slslo CR 48, in Peconic; and
WHEREAS on August 03, 2004, an amended site plan application was submitted for
approval; and

WHEREAS on October 4, 2004, the South old Town Planning Board, acting under the
State Environmental Quality Review Act (6 NYCRR), Part 617.5@ (3), made a
determination that the proposed action is a Type II and not subject to review; and
WHEREAS, on October 07,2004, the Architectural Review Committee reviewed the
architectural drawings and associated site plan materials in accordance with its
recommendations of August 17, 2004, and approved them with conditions; and
the Planning Board has accepted the conditions and determined them to be
satisfactory; and
WHEREAS, on October 13, 2004, the South old Fire District found adequate fire
protection for this site plan and indicated this recommendation is valid for one year; and
the Planning Board has accepted this recommendation; and

WHEREAS, on October 14, 2004, the Southold Town Building Inspector reviewed and
certified the site plan; and _
WHEREAS, on December 6,2004, the Southold Town Engineering Inspector reviewed
and approved the proposed drainage and the Planning Board has accepted his
recommendation; and
WHEREAS, on December 10, 2004, the Suffolk County Farmland Committee approved
the site plan prepared by Samuels & Steelman Architects and certified by Nancy L.
Steelman, Registered Architect, dated July 23, 2004 and the Planning Board has
accepted the approval; and
WHEREAS, the Southold Town Planning Board, pursuant to Chapter 58, Notice of
Public Hearing, has received affidavits that the applicant has complied with the
notification provisions; and
WHEREAS, the following two items are incorporated and included in the site plan:
1. All outdoor lighting shall be shielded so that the light source is not visible from
adjacent properties and roadways. Lighting fixtures shall focus and direct the
light in such a manner as to contain the light and glare within property
boundaries. The lighting must meet thE'_Town Code requirements.
2. All signs shall meet Southold Town zon,,1g codes and shall be subject to
approval of Southold Town Building Inspector; and
WHEREAS, all the requirements of the Site Plan Regulations of the Town of Southold
have been met; be it therefore
RESOLVED that the Southold Town Planning Board grant final approval on the site plan
prepared by Samuels & Steelman Architects and certified by Nancy L. Steelman,
Registered Architect, dated July 26,2004, and the Drainage Plan dated October 27,
2004, and authorize the Chairperson to endorse the final site plans with the following
condition and subject to a one-year review from the date of issuance of the building
permit:
1. All ground and/or building-mounted signage shall be submitted to the Planning
Board for review prior to installation.
These requirements must be met within six (6) months of the resolution. Failure to
adhere to these requirements within the prescribed time periods shall render this
approval null and void.
