Dear Ms. Neville:
The Mattituck Park District has commenced the approval processes for construction
and financing of a proposed building at Veteran's Memorial Park.
Enclosed is a copy of a resolution of the District adopted at a meeting on Sept. 8,
2001, by which it was resolved that the District start the lead agency coordination process
on this unlisted action. Also enclosed is a copy of the Sort Environmental Assessment
Form.
The Park District would like to continue as lead agency and issue a declaration under
the SEQRA process. In the event the Town prefers itself or another agency to have lead
agency status, or in the event the Town has any comment with respect to the declaration,
please advise. I have previously discussed this matter with Mr. Yakaboski.
This notice is also being given to the Suffolk County Department of Health Services
and the N.Y.S. Department of Environmental Conservation.

The undersigned, the secretary of the Mattituck Park District in the town of Southold,
Sufi:blk County, New York, (the "Park District"),
DOES HEREBY CERTIFY AS FOLLOWS:
At a meeting of the Board of Commissioners (the "Board") of the Park District, duly
called and held this day at which a quorum was present and acted throughout, the
Commissioners unanimously adopted the following resolution, which has not been .
modified or rescinded:
WHEREAS, the Park District is considering the construction of a building at the
property heretofore acquired and now owned by the Park District at Veteran's
Memorial Park, Peconic Bay Avenue, Mattimek, New York, in the Park District, for
use as a storage and maintenance build'rog for its equipment and two offices; for a
concession stand; and for bathroom facilities (the "Facility), which Facility would be
substantially in accordance with the site plan prepared by Warren Sambach dated
June 1, 2000, as same may be amended from time to time; and
WHEREAS, the Park District would like to commence the review and action
necessary under Part 617 of the implementing regulations pertaining to Article
(State Enviromnantal Quality Review Act) of the Environmental Law (SEQKA),
NOW, THEREFORE, upon motion duly made and seconded, discussed, and carried,
it was RESOLVED,
A. That the Park District determines that the project is an unlisted action under
SEQRA.
B. That the Park District declares itself as lead agency and hereby resolves to
start the lead agency coordination process on this unlisted action under SEQRA.

Thursday, August 2~t, 1995 the following action was taken:

Moved by Stephen Angell, seconded by Betty Wells, it was
RESOLVED to recommend to the Southold Town Board of Trustees APPROVAL
of the Wetland Application of MAI IITUCK PARK COMMISSION 126-6-17 & 18
to reconstruct existing deteriorated timber jetty (125' in length) in kind/in
place.
The CAC recommends approval but questions why the application fee is being
waived. It appears inappropriate that the Park District fee should be waived
since it is not beneficial to all town residents.

Dear Mr. Bredemeyer:
In the December, 1992 storm, a portion of the bulkhead
adjacent to Mattituck Park District Memorial Park site on
the corner of Peconic Bay Boulevard and Bay Avenue in
Mattituck, was destroyed. During the storm the loss of a
natural dune area which was supported by said bulkhead was
leached out into Peconic Bay leaving 300, more or less feet
of deteriorated bulkhead on the west side of this eight acre
parcel.
We are requesting as Commissioners, that the Town
Trustees issue an emergency permit allowing us to remove the
portions of this deteriorated bulkhead as we had done in the
storm Nelson in 1983. Since funds are not available at this
time to re-build said bulkhead, it is necessary for
liability purposes and the opening of the beach in late
spring, to remove same as soon as possible.
I realize that this request is a little dated. However,
we have been working with the D.E.C. in getting an emergency
permit also.
If you would like any pictures, we would be glad to
furnish same.
