I would like to take this opportunity to advise you of my change of plans regarding the subject
Building Permit 31218 Z granted June 22, 2005.
I have elected to downsize lhe project and the only work which will be under taken will be the
addition of the enclosed porch on the north side of the house. I have completed the excavation for
the piers and will be calling for a foundation3 inspection when the forms are in place.
The mom will use new windows since the windows originally planned to be removed [~. the
house and reinstalled on the porch will net be available. The door on the w~st will be eliminated.
The layout of the beams and floor joists has been rotated to accommodate the locations of the
bulkhead tie rods, anchor piles and lay piles.

All applicants for permits* including Town of Southold agencies, shall complete this CCAF for
proposed actions that are subject to the Town of Southold Waterfront Consistency Review Law. This
assessment is intended to supplement other information used by a Town of Southold agency in
making a determination of consistency. *Except minor exempt actions including Building Permits
and other ministerial permits not located within the Coastal Erosion HazardArea.
Before answering the questions in Section C, the preparer of this form should review the exempt
minor action list, policies and explanations of each policy contained in the Town of Southold Local
Waterfront Revitalization Program. A proposed action will be evaluated as to its significant
beneficial and adverse effects upon the coastal area (which includes all of Southold Town).

affect the achievement of the LWRP policy standards and conditions contained in the consistency

permitted bulkhead and west of the marine mattresses with sand, and to repair the east
side of the access road by placing rocks, moved by the storm, in suitable mesh
containment, and filling with soil and planting vegetation to stabilize the soil.

It is my understanding that under ,Nticle 97-13, Exceptions, Paragraph A, Item 5 of the new Chapter
97 Wetlands and Shoreline' of the Code of the Tow~ of SoulY~ld that the chapter shall no~ affect or
prohibit nor require a permit for the repair.
The repair will be accomplished ~, adding a layer of Z' x 10" Tongue and Groove Plastic Sheathing to
the existing bulkhead where the damage has occun'ed.
On April 6, 2004 I stopped in the Trustees Office and spoke to ~ Standish and she agceed a
permit was not required.

WHEREAS, GERARD H. SCHULTHEIS applied to the Southold Town Trustees for a permit

Town of Southold, application dated March 28, 2003, and,
WHEREAS, said application was referred to the Southold Town Conservation Advisory Council
for their findings and recommendations, and,

April 30, 2003, at which time all interested persons were given an oppommity to be heard, and,
WHEREAS, the Board members have personally viewed and are familiar with the premises in
question and the surrounding area. and,
WHEREAS, the Board has considered all the testimony and documentation submitted
concerning this application, and,
WHERIEAS, the structure complies with the standards set forth in Chapter 97 of the Southold
Town Code,
WHEREAS, the Board has determined that the project as proposed will not affect the health,
safety and general welfare of the people of the town,

RESOLVED, that the Board of Trustees approve the application of GERARD H.
SCHULTHEIS to demolish the unstable east wing and build a new addition where east wing
exists and add a second-story to northwest section of house. Replace roof over porch on north
side with new roof. All building to be performed within existing building envelope, using
existing foundation, and subject to the same conditions as the prior Permit #5620, in terms of
fencing prior to and during construction, and the installation of drywells and gutters for roof run-
off control.
Permit to construct and complete project will expire two years from the date the permit is signed.
Fees must be paid, if applicable, and permit issued within six months of the date of this
notification.
Inspections are required at a fee of $5.00 per inspection. (See attached schedule.)
