A. For new building or new use: 1. Final survey of property with accurate location of all buildings, property lines, streets, and mmsual natural or
topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal (S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plmnber certifying that the solder used in system contains less than 2/10 of 1% lead.
5. Cmmnercial building, industrial building, multiple residences and similar buildings and installations, a certificate
of Code Compliance from architect or engineer responsible for the building.
6. SuNmt Plamfing Board Approval of completed site plan requirements.

a. This application must be completely filled in by typewriter or in ink and submitted to the Building Inspector, with 3
sets of plans, accurate plot plan to scale. Fee according to schedule.
b. Plot plan showing location of lot and of buildings on premises, relationship to adjoining premises or public streets
or areas, and giving a detailed description of layout of property must be drawn on the diagram which is part of this appli-
cation.
c. The work covered by this application may not be commenced before issuance of Building Permit.
d. Upon approval of this application, the Building Inspector will issued a Building Permit to the applicant. Such permit
shall be kept on the premises available for inspection throughout the work.
e. No building shall be occupied or used in whole or in part for any purpose whatever until a Certificate of Occupancy
shall have been granted by the Building Inspector.

Subject: Field Inspection of Existing House for Conformance with Design Drawings By
Howard Patterson Architects Dated 12/8/83
Lot 2-2, Fishers Island, NY
Dear Mr. and Mrs. Travers:
As requested, an engineer from CME has performed a visual site inspection of the subject house. The
purpose of the inspection was to determine if the house was constructed in accordance with design plans
by Howard Patterson Architects dated 12/8/83. The site inspection was limited to structural elements that
were visible in the house at the time of inspections.
Based on our visual inspection we have determined that house has been built in general conformance with
the design plans by Howard Patterson Architects dated 12/8/83. The floor layout is as shown on the
design plans with the exception of the porch not being constructed off of the front entryway. There are
four bedrooms, 2 full bathrooms and a laundry room on the first floor. The second floor consists of a,
living room, dining room, kitchen, bedroom and bathroom. Finally, the attic space is being used for
storage.
I have also included some photographs taken during the inspection for your review.

permanent ladder instead of pull down stairs. Finally, the foundation is a full basement
whereas the design plans show a crawl space.
I have forwarded final survey plans showing the septic system location to the Health
Department and am awaiting their final approval. I am forwarding this letter along with
the certificate from the plumber to the Building Department. I have also included some
photographs taken during the inspection for your review.

sink as you face window from Kitchen.
7. Add one winder to stair landing (3 winders total). This will
eliminate one stair tread in ~all.
8. Move north wall of Entry Rall as discussed in field. Also
relocate doors es discussed in field.
