A. For new building or new use: 1. Final survey of property with accurate location of all buildings, property lines, streels, and unusual natural or
topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal (S-9 form).
3. Approval of electrical installation fi.om Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains less than 2/10 of 1% lead.
5. Conmlercial building, industrial building, multiple residences and similar buildings and installations, a certificate
of Code Compliance from architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
B. For existing buildings (prior to April 9, 1957) non-conforming uses, or buildings and "pre-existing" land uses:
1. Accurate survey of property showing all property lines, streets, building and unusual natural or topographic
features.
2. A properly completed application and consent to inspect signed by the applicant. If a Certificate of Occupancy is

GENERAL DESCRIPTION:
An inspection was made to the existing buildings off of Sage Boulevard on October
13, 1995. The buildings were evaluated for structural repairs required to make each
building sound. The buildings were evaluated in sections: foundation, floor framing, interior
and ex~terior wall framing, and ceiling and roof framing. The following is a break down of
each of those components as a percentage of the whole structure:
Foundation 30 %
Consists of exterior and interior masonry of wood supports
Floor framing 20%
All main girders, floor joists and sub flooring
Exterior and interior wall framing 25 %
All sills studs plates and headers, and sheathing
Ceiling and roof framing 25%
Ceiling joists, roof rafters, and sheathing

No evaluation of plumbing, or electric will be detailed in this report. Also cosmetic
repairs, such as interior surfaces, broken widows, or window sash, or other interior or
exterior finishes will not be commented upon.
