Dear Sir,
I inspected the detached barn this date for Odden/Lampas at 315 Rachel's Road. The barn is a
mid 19th century barn that has had extensive repairs in recent years. The barn is in need of additional
repairs to keep the structure from deterioration especially in the valley rafter where insect infestation was
noted. But as of the time of the inspec~te the barn was in sound condition.

This application must be filled in by typewriter or i~. and submitted to th~Building Deparrmeut with the following:
For new building or new use:
l. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or
topographic features.
2. Final Approval from Health Dept. of water supply and se;verage-disposal (S-9 fbnn).
3. Approval of electrical installation from Board of Fire Underv,,riters.
4. Sworn statement from ptumber certifying that the sotder used in system contains less than 2/10 of 1 ob lead.
5. Commercial building, industrial building, multiple residences and similar buildings and installations, a certificate
of Code Compliance from arci~itect or engineer responsible for the building.
6. Submit Plam~ting Board Approval of completed site plan requirements.
For existing buildings (prior to April 9, 1957) non-conforming uses, or buildings and "pre-existing" land rises:
l. Accurate survey of property showing all property lines, streets, building and unusual natural or topograpbic
features.
2. A properly completed application and consent to respect sigmed by the applicant. Ifa Certificate of Occupancy is
denied, the Building Inspector shall state the reasons theretbr in writing to the applicant.
