Re: Anthony and Madeline Pisciotta, 8554 Main Bayview Road~ Permit 4934
Dear Mr. Krupski:
The captioned permit was issued 25 June 1998 and expired 25 June 2000. Sadly, however,
Anthony Pisciotta has passed away and Mrs. Pisciotta has not had the opportunity to follow
through with the approved project in any manner.
As it appears to be too late to extend the Trustees' permit, please find attached a new application
by Madeline Pisciotta for the same project approved by the Board two years ago. Please note
that the plan remains unchanged in all regards, and that the Pisciottas' New York State
Department of Environmental Conservation and Suffolk County Department of Health Services
remain valid and current, copies of which are attached to the application.
