I inspected the foundation today and found that the work was done according to code. The location of the anchor bolts follows the architect's drawing. The top three courses of blocks were filled solid with mortar. The walls are properly plastered and dampproofed. The contractor provided the necessary access opening (ca 16"x24")into the existing crawl space and An additional ca 16"x8" vent opening for proper ventilation. I hereby state that the information provided above is true to the best of my knowledge.
