This application must be filled itt by typewriter or ~ and submitted to tile Building Department with tile following:
For new building or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or
topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal (S-9 form).
3. Appro'cal of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains less than 2/10 of 1% lead.
5. Commercial building, industrial building,.multiple residences and similar buildings and installations, a certificate
of Code Compliance from architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
For existing buildings (prior to-April 9, 1957) non-conforming uses, or buildings and "pr~-existing" land uses:
1. Accurate survey of property showing all property lines, streets, building and unusual natural or topographic
features.
2. A properly completed application and a consent to inspect signed by the applicant. If a Certificate of Occupancy
is denied, the Building Inspector shall state the reasons therefor in writing to the applicant.

In accordance with the Agreement between the Town of Southold and Sidney B. Bowne & Son,
LLP, Consulting Engineers, dated JanuatT 14, 2000, for professional engineering services
relating to the above project, we have completed the final inspection for the subject premises.
In addition, we have reviewed inspection reports and other relevant material for conformance to
the requirements of the Town Building Department for a Certificate of Occupancy.
Based on this review, we have no objection to the issuance of a Certificate of Occupancy for the
subject unit.

Apartment Buildings & Parking Garage for 200lbs at txvo hours at the above referenced project as requested by
E & F Walsh Building Company, LLC. STAT Fire Suppression. Inc. has designed and installed the Fire Sprinkler
System in accordance with NFPA #13R and local authority codes.
At this thne STAT Fire Suppression. Inc., has found the system to be in satisfactory condition and free of
leaks. STAT Fire Suppression, Inc., will not be held responsible for any damage done in these areas. An).
