INSTRUCTIONS
A. This applicaticn to he completely filled in by typeWl'iter or in ink and submitted to the Town Trustees in dup-
licate.
B. Plot plan òhowing location of lot and buildings on p,'emises, relation to adjoining premises, public streets,
parks, etc., and giving detailed de3cliption of prepcsed layout of project must be drawn on diagram which is part of
this applica;tion.
C. The work covered by this application may not be commenoed before the issuance of a permit.
D. Upon approval of this application, Town Trnstee. will issue a permit to applicant. Such permit shall be kept
on the promises and availa!>le fer inspection at all times.
E. Applicant shall notify the Bca','d of Trustec3 u'!en cOh:;.'~ction of the work covered by permit.

Trustees or its agents.
