revocation of this Pennit by resolution of the said Boord.

work shall cause unreasonable obstruction to free navigation, the said Permittee will be required,
upon due notice, to remove or alter this work or project herein stated without expenses to the Town
of Southold.
8. That the said Board will be notified by the Perminee Ot the completion of the wotk auth-
orized.
9. That the Permittee will obtain IU other permits and ronsents dJat may be requited sup-
plemental to this permit which may be subject to revoke upon failure to obtain woe.

Dear Mr. Hall:
The following action was taken by the Board of Town Trustees
during its regular meeting held on February 24, 2000 regarding
the above matter:
WHEREAS, Land Use Ecological Services, Inc. on behalf of WILLIAM
A. MALLINS applied to the Southold Town Trustees for a permit
under the provisions of the Wetland Ordinance of the Town of
Southold, application dated October 7, 1999, and,
WHEREAS, said application was referred to the Southold Town
Conservation Advisory Council Ïor their findings and
recommendations, and,
WHEREAS, a Public Hearing was held by the Town Trustees with
respect to said application on February 24, 2000 at which time
all interested persons were given an opportunity to be heard,
and,
WHEREAS, the Board members have personally viewed and are
familiar with the premises in question and the surrounding area,
and,
WHEREAS, the Board has considered all the testimony and
documentation submitted concerning this application, and,
WHEREAS, the structure complies with the standard set forth in
Chapter 97 of the Southold Town Code,

WHEREAS, the Board has determined that
will not affect the health, safety and
people of the town,
the project as proposed
general welfare of the
NOW THEREFORE BE IT,
RESOLVED, that the Board of Trustees approve the application of
WILLIAM A. MALLINS to construct a 4'X 40' fixed timber dock,
elev. 4' above marsh, a 3'X 20' ramp and a 6'X 20' float. The
whole dock will have 12-8" piles overall, with the condition
that the entire structure is not to extend no further seaward
than the neighboring docks.
BE IT FURTHER RESOLVED that this determination should not be
considered a determination made for any other Department or
Agency which may also have an application pending for the same
or similar project.
Permit to construct and complete project will expire two years
from the date it is signed. Fees must be paid, if applicable,
and permit issued within six months of the date of this
notification.
Two inspections are required and the Trustees are to be notified
upon completion of said project.
Fees: $660.00

Dear Mr Krupski:
Enclosed please find (7) sets of plans for the above referenced project. These plans have been
amended to reflect a revised 50' buffer area. This revised buffer area is now setback from the Tidal
wetlands line as opposed to average high water At the 12-15-99 Trustees hearing it was decided that
this would suffice for the approval of the residence. The Dock will be restaked in the proper location for
inspection by the Trustees.

The following action was taken at the Southold Town Board of
Trustees Meeting held on Wednesday, October 25, 2000:
RESOLVED, that the Southold Town Board of Trustees grant the
request for an Amendment to Permit #5116 to install a permanent
4'X 40' elevated catwalk, 4'X 20' ramp, 3'X 5' float and 6'X 20'
float to be secured by two 8" piles, with the condition that,
including the vessel, it is not to be more than 1/3 across the
width of the creek or more than 50' seaward of the vegetated
wetlands, which ever distance is the least.
This is not an approval from any other agency.

At the meeting of the Southold Town Conservation Advisory Council held Monday, October 23,
2000, the following recommendation was made:
Moved by Melvyn Morris, seconded by Scott Hilary, it was
RESOL VED to recommend to the Southold Town Board of Trustees DISAPPROVAL of the
Amendment Request of WJLLL<\M A. MALLINS 113-4-3&4 to Amend Permit #5116 to
install a permanent 4'X 40' elevated catwalk. 4'X 20' ramp, 5'X 5' float and 6'X 20' float to be
secured by two 8" piles.

The following action was taken by the Board of Town Trustees during its Regular
Meeting held on Wednesday, Decembe,' 15, 1999 regarding the above matter:

Trustees Meeting held on Wednesday, April 19, 2000.
RESOLVED, that the Southold Town Board of Trustees grant the
request for an Amendment to Permit #5116 for a 3'X 30' seasonal
ramp, a seasonal 6'X 20' float and 4"X 4" piles for the dock and
4"X 6" piles for the float.

Enclosed please find (7) sets of revised plans and project descriptions for the above referenced
project. These plans have been amended as per the request of the NY.S.DEC. and include the
following changes:
- Proposed ramp is now 3' x 30' and seasonal.
- float is now proposed to be seasonal.
- 4"x 4" piles are now proposed for the dock, and 4" x 6" piles for the float.

Project Description:
Applicant proposes to construct a single family dwelling with a garage,
deck, pervious stone drive, sanitary system, and a recreational dock facility. Proposed
house and garage is proposed to be 3,285 s.f., proposed pervious drive is proposed to
be 1,100 s. f. Proposed house is proposed to be setback a minimum of 75' from the
Tidal Wetlands Line, with a 50' buffer area of non-disturbance. The proposed sanitary
system is proposed to be setback 125' from the Tidal Wetlands Line. The total upland
area for the project site is 45,621 +/- s. f., the proposed impervious structure area for
this proposed project is 3,745 s.f. yielding a percent adjacent area coverage of 8.2%. A
proposed 1000 +/- cubic yards of fill is required for the proposed residence and
sanitary system. Proposed residence will utilize public water.
Applicant also proposes to construct a recreational dock facility consisting
of the following: 4' x 40' Fixed CCA Timber Dock, 3' x 30' Ramp (SEASONAL), and 6' x
20' CCA Timber Float (SEASONAL). Proposed facility will utilize (10) 4" X 4" CCA
Timber Piles for the fixed dock and (2) 4" X 6" piles to support the float. The fixed dock
is proposed to be elevated a minimum of 4' above the existing wetlands vegetation. A
proposed 4'+/- natural woodchip/mulch path is proposed to access proposed dock
facility .

Project Description:
Applicant proposes to construct a single family dwelling with a garage
and an asphalt drive, sanitary system, and a recreational dock facility. Proposed house
and garage is proposed to be 3,465 s.f., proposed asphalt drive is proposed to be 750 s.f.
Proposed house is proposed to be setback a minimum of 75' from the Tidal Wetlands
Line. The proposed sanitary system is proposed to be setback 125' from the Tidal
Wetlands Line. The total upland area for the project site is 45,621+/ - s.f., the proposed
impervious structure area for this proposed project is 3,465 s.f. yielding a percent
adjacent area coverage of 9.2%. A proposed 1000 + / - cubic yards of fill is required for
the proposed residence and sanitary system. Proposed residence will utilize public
water.
Applicant also proposes to construct a recreational dock facility consisting
of the following: 4' x 40' Fixed CCA Timber Dock, 3' x 20' Ramp, and 6' x 20' CCA
Timber Float. Proposed facility will utilize (12) 8" DIA. CCA Timber Piles, (10) Piles for
the fixed dock and (2) piles to support the float. The fixed dock is proposed to be
elevated a minimum of 4' above the existing wetlands vegetation. A proposed 4'+/-
natural woodchip/mulch path is proposed to access proposed dock facility.
