Request for Additional Information
On October 18, 1999 the Department of State (NYSDOS) received your consistency certification and supporting
information regarding the proposed expansion of an existing dock by the addition of a ramp and float. We are
concerned with various items and requests additional information. Pursuant to 15 CFR Part 930.58, the following
data and information is necessary to enable the Department of State to adequately assess the coastal zone effects of
this proposed project.
Your submitted site map illustrates that Haywater Road exists between your client's parcel and the dock. Does your
client have permission or an easement from the owner of the parcel that provides access to the dock? In addition,
it appears that the proposed float xvould be constructed/encroach on a neighboring parcel. Please submit
documentation that clearly illustrates ovmership of the parcel by your client or authorization from the owner for the
construction of, and access to the dock.
Pursuant to 15 CFR 930.60(a), the review of your consistency certification wilt begin after we receive and determine
that the necessary data and information is adequate to assess coastal zone effects. Any additional information which
you believe will facilitate our consistency review of this project would be appreciated.
A copy of this letter has been sent to the U.S. Army Corps of Engineers.

At the meeting of the Southold Town Conservation Advisory Council held Monday, July 19,
1999, the following recommendation was made:
Moved by Jaclyn VanBourgondien, seconded by Richard Smith, it was
RESOLVED to recommend to the Southold Town Board of Trustees DISAPPROVAL of the
Wetland Permit Application of GERALD GELBWAKS 111-1-23.1 & 28.1 to attach a YX 12'
timber ramp and 6'X 20' float secured by (2) 8" diameter piles to existing timber dock.
Haywater's Rd./Fisherman's Beach Rd., Cutchogue
The CAC recommends Disapproval because the proposed structure in addition to the existing
