Box 1179 Southold, New York 11971 Attn: Lauren Standish Re: Preliminary Site Visit to inspect site conditions for an addition and septic system upgrade, Madeline Avenue, Fishers Island (1000-6-7-12) To the Town of Southold Trustees: I am writing to you on behalf of Mrs. Louise Packard regarding a proposed addition to her single-family house on Fishers Island. I have enclosed a sketch of her site showing the proposed expansion and approximate location of the new septic system. Would it be possible for the trustees to visit the site during their visit to the island this Wednesday to familiarize themselves with it in anticipation of a complete application later this fall. Because there is virtually no upland outside of the buffer zone some consideration will have to be given to the new septic system. Mrs. Packard has copied me with her authorization to visit the property. If you have not received this letter, please call me to let me make arrangements for you to get a copy. If you have any questions or require any additional information, please call. Thank you for your attention to this matters. Box 1179 Southold, NewYork 11971 Re: Permitting for an existing addition to a single family residence at Madeline Avenue, Fishers Island To the Town of Southold Trustees: I am writing to you on behalf of Mrs. Louise Packard regarding an existing addition to a single-family house on Fishers Island. Enclosed please find a set of permit applications (CEHA, General Data, Wetlands and Land Trustees Permit, Long & Short Environmental Assessment Forms) and a check in the amount of $50.00. In addition, I have enclosed several copies of the as-built drawings as well as several photographs of the property. Mrs. Packard requests a permit for an as-built sun room and porch on her house. In the process for applying for a new building permit, it has come to our attention that prior to Mrs. Packard purchasing her share of this property, a small family room was added without obtaining the proper permits. As requested by the building department in a memo dated December 17, 1999, we have been instructed to apply for both a Trustees Permit and a DEC permit, which is also complete, for the as-built conditions at the site before Mrs. Packard can continue with her proposed alterations. Mrs. Packard wishes that the proper channels were followed when the addition was constructed and is hopeful that this application will correct those oversights. Please feel free to contact me at days (508) 993-6567 or evenings (508) 758-3206, if you have any questions or require any additional information. Thank you for your attention to these matters.
