A. INSTRUCTIONS
1. All applicants for permits* including Town of Southold agencies, shall complete this CCAF for
proposed actions that are subject to the Town of Southold Waterfront Consistency Review Law. This
assessment is intended to supplement other information used by a Town of Southold agency in
making a determination of consistency. *Except minor exempt actions including Building Permits
and other ministerial permits not located within the Coastal Erosion Hazard Area.
2. Before answering the questions in Section C, the preparer of this form should review e-exempt
minor action list, policies and explanations of each policy contained in the Town Pouthold Local
Waterfront Revitalization Program. A m osed action will :be evaluated to its si "icant
beneficial and adverse effects upon,the coastal area which includes all of Sq old Town .
3. If any question in Section C on this form is answered "yes" or " o", then the proposed action will
affect the achievement of the LWRP policy standards and nditions contained in the consistency

Toscano Project Description
Establish a 10 ft. long x 4 wide pathway through Trustee non turf area to DEC
non disturbance area. Then construct a 40 ft. long x 4 ft. wide raised walkway
160 SF ) w/ staircase ( 24 SF ) through DEC non disturbance area to wetland
boundary which includes 15 ft. of the remaining Trustee non turf buffer and 25
ft. trustee non disturbance buffer. Then construct a 40 ft. long x 4 ft. wide raised
walkway ( 160 SF ) through DEC and Trustee non disturbance area to MHW line .
Total length of raised walkway through DEC and Trustee non disturbance zones
to HWM is 80 lin ft. ( 320 SF ). Construct a 46 ft. x 4 ft walkway ( 184 SF )
Install a 3 ft. x 12 ft. aluminum ramp from fixed walkway to floating dock ( 36
SF, 24 SF over water ), install an 18.7 ft. x 6 ft floating dock w/ a 2 ft. x 4 ft.
bump out for ramp ( 120 SF ) w/two (2) sets of dauphin pilings on ends for
stability. Walkway to have thru flow decking throughout w/ pressure treated
pilings set at 8 ft. O.C. Total length of walkway is 126 lin. ft. ( 504 SF ).
