Disease Laboratory, and his colleague, Dr. Jerry walker, appeared before the Board at the request of Supervisor Martocchia as a result of a letter from .Edward Sqhwindt, Jr. re.la~ive to the concern of a group of local citizens about plans by Plum Island to begin research on Rift Valley. Fever. Dr. Callis stated that since the inception of the laboratory facilities in 1954, their mission has been diagnostic and research work on any animal disease which occurs anywhere in the world. Rift Valley Fever is a disease which has been on their list to work with since Plum Island started. Rift Valley Fever has moved from Africa, its usual haunt, to Sudan, to Egypt, and is now in the Nile Delta bordering on the Meditteranian. since it has moved from Africa, they find it a threat to U.S. livestrock. There have been three things determined which must be done to protect U.S. livestock: 1. they must develop diagnostic competency, an antigen and anti- serum must be produced on Plum Island. 2. capability of vaccine for immunizing livestock must be developed. There is a vaccine which has been used widely to protect man, but has never been tried in live- stock. 3. Determine the roll~ of the horse; is it susceptible ito Rift Valley Fever? Since plans have developed to work on Rift Valley Fever over a period of several months, employees who will in any iway work with it are being vaccinated. Twenty have already been vaccina- ted and 70 more are being vaccinated today. The vaccine is not FDA approved because there has never been a demand for it by the public. The vaccine was developed by the military for troops who would be stationed in foreign countries. There will be many safeguards taken in working with Rift Valley Fever; it will be worked on only under hoods, and the people working with live animals will have stringent safeguards. The work will take about 90 days, commencing in January and should be completed by the first of April. Plum Island has informed local municipalities, local medical people, the Suffolk County Me'dical S___ociety and the New York State Medical Society of their plans. The incident on Ssptember 15, 1978 involving an outbreak of foot and mouth disease is presently ~inq investi~ated~ There should be an answer to the cause by this coming Friday. However, they think it is in someway connected with the construction program where n~rmal working procedures must be changed to accomodate the construction. ..... Laboratory. to address the audience relative to Rift V~!1¢¥ Fever. The invitation of Dr. Callis to be at this meeting came about because of a letter received by Supervisor Mart0cchia from Edward Schwind~ JR.~ as he did not feel qualified t~answer Fir. Schwindt's questions. Dr. Callis' statements were ~ch'the same as s~ated at the executive session.-~that Plum Island has be~n doing the same thing today they started :out todo 25 yeams ago. Basically they do two things. They operate a diagnostic laboratgry which is the first part of their mission, and in that ~. since ~'ey~ reference la~orato~y f~ ~the:res~ of the Un,ted Sta~s% They operate a diagnostic~ laboratory and must be qualified Wnd ~able and have the materials with which~to diagnose an animal di~ ~ha~ ~ight-occu~ &nypl~ce in the wo~ld. That has been their mission since 1954 and remains their mission. There is a listing of about 40 animal diseases with which they are concerned. They are concerned on Plum Island with domestic animals, but their primary mission is foreign animals. They are concerned about domestic animal diseases only as they relate to ~the foreign ones and %e. be a~le-to differentially diagnose them. When~ they started in 1954, they perhaps got three samples a y.ear., this y~ar they are probably going to receive one a working day ar five a week or about 220 to 250 Per year. That is an indication of how this phase of what they d~!has changed. Rift Valley Fever, which they have become concerned with, has been listed in booklets describing Plum Island since 1956; the first addition in 1963, the second addition and the most recent one 'published in 1976. They bOugh~ about~ 10,000 of them and most of them are gone. They list the animal diseases with which they are concerned, which they have to be capable of diagnosing. Rift Valley Fever is right on page one. They have not worked with all 40 animal diseases Which are in their charter and ~ith which the~.are,concerned because they have not had time in the 25 years they.have been in exlstence to work with all 40. They have worked with approximately 24. ' The diseases on the list o~ which ~hey are going to work. and~at which time varies from time to time depending upon many factors,~the primary factor being what are ~he hazards, or risks~today of Rif%~ Valley Fever coming into the united States and infecting our 200 million livestock. There ~s 116 m3~llion cattle, 60 mi'ilion pigs, 8 million horses, mules, asses and donkeys, 20 million sheep and goats. Of those species, the cattle,sheep and goats are susceptable to Rift Valley Fever. Rift Valley Fever is an animal disease, the virus also infects man, but it is an animal disease They work with many diseases, agents which infect man,.~his is really nothinq_ new. Dr. Callis spoke aboutmany di,seases.~hich infect man and which are studied inthe United States. The disease with which they work on at any given moment varies depending upon their assessment of the risk that that disease poses to our Valuable livestock industry. How do they~assess and evaluate that? Many methods. First of all they read sc.len'ti~ic journals, all of this information is published in the open l~iterature. They get information from animal disease movements by people in the State Department, primarily the Agricultural Attache which we.have_ in every Embassy around the world. They attend meetings and t~lk to their colleagues and there are international organization's that make a business o'f disseminating the information; they also get information on animal, movement from'the United Nations. Why did Rift Valley Fever-.mov~ up on their priority? Very simply. In their assessment of Rift Valley Fever 25 years ago, they put it in the l~ower 25%. ~ift Valley Fever gets its name from the fact it is a disease first described in animals in the Rift Valley which is a valley which goes up and down thJ east coast of Africa r~ght on into the Middle East, formed millions of years ago during the glacier periods. The disease has traditionalZy gone back and for.th between South Africa and Kenya. Never been further north than Kenya until 1976. For some unknown reason, in 1976 this disease went into the Sudan, in 1977 it wentto Egypt, and at this moment, it 'is all the way to the Mediterranian in the Nile Delta, the Nile Valley. So, yes, they are now concerned about Rift Valley Fever, whereas pre- viously they ~were not. The Israelis ~re very concerned about Rift Valley Fever, they get one or two telephone calls a week from their colleagues in Israel who want to know about sampling techn~.qU~s and they say they are sorry, they never worked on Rift Valley F~ver, call South Africa. Well, their relations with South Africa are not very good so they call Kenya where their relations, thankfully, are good. The opinion is expressed in Mr. Schwindt's letter that Plum Island should do their work in Egypt. They are working in Egypt, they have a cooperative agfeemen~ with the Ministry of Agriculture in Egypt. The person on their staff responsible for that is Dr. Walker' There are' three things that they have determined should be done on Plum Island which cannbt be done in Egypt. 1. They do run a diagnostic laboratory and they'must work with an animal disease in order to be qualified to diagnose it. They took the position a good many years ago that they were not qualified to diagnose an animal disease until they worked on it in their facilities on Plum Island and until they produced the necessary diagnostic materials, antigen and anti-serum, standardized those products, safety tested them and put them in the refrigerator or ~eep freeze and have them available at a moments notice no~ only for use in their laboratory but in the event that Cornell or the University of California or the University of F~orida might request this diagnostic material. They have to have t6 have it ready and avazlable to send out at a moments notice and it must be done safely, so they changed their emphasis on Rift Valley Fever due to the fact the disease has moved, the picture has changed. In the past twenty-five years, they had' concluded it wasn't much of a risk, right at the moment and over the last two years, they think it is quite a risk. He does not think that Rift Valley Fever, if it gets into the United States, is going to get here by way of an animal. If Rift Valley Fever gets into the United States it is going to get here by way of somebody getting of~ of an airplane at Kennedy Airport. This disease can be transmitted by man and the v~rus in the blood stream. So they have concluded that they must work on Rift Valley Fever to protect our livestock industry. They have to know how to diagnose, have to produce diagnostic materials. They have been in the planning stages of this for some time and are still in the planning stages of it. The first thing they must do is immunize the people. I~t~is an animal d~isease but it can infect man, so the people who are going tQ-be exposed to the virus on Plum Island must be pro- tected. -Some of them ~have already been immunized, he has been, Dr. walker has been, and,over the last several months, they have identi- fied 90-~people on Plum Island who should be i~unized, who have any reason-whatever to go into the building called Diagnostic Investiga- tions. The submission to the immunization process with this vaccine is~?~oluntary on the part of the Plum Island employees. They indica- ted there are 90 people who should be immunized. Information on the disease-was disseminated on Plum Island, Dr. Walker and or Dr. Callis spokei~o, every per'son on Plum Island and fully informed them about what they were doing. They disseminated all the information that is available ~to them, about the disease, about the vaccine and they receivedback 90 slips indicating that the 90 people were willing to take-the-immunization. T~his is a ~oluntary immunization because of ,the fact; the- ~acc~ne is not approved'by the Food and Drug Administra- 'tion.. The reason the vaccine ~s not approved by the Food and Drug A'dministration is because there ha~s never been a need for this facci~e .on a ~ide scale use in the Un~ted States. This .vaccine was developed by the military to protect ~roops ~ho may have to gO any~ -~place in the world to protect U.S. citizens for U~S. interests. It has ~een used~ however, in literally thousands of people and he does not think he has-ever had an~ immunization or inoculation that was so innocuous, he simply forgot he had it, and it has been the experience of all of the people who have had it. Dr. Callis stated that, in their _latest planning believe th&t the work on Rift Valley, Fever~ can begin approximately the middle of December or January and there is no reason whatever why it cannot be finished by the first of April~ or 90 days after they begin. It shouldn't take any longer to do the three things which he mentioned - This will be a time when mosquitoes are not very viable on Long Island. ~his is not the first disease with which they have worked which is transmitted by mosquitoes. They-have been working with African Horse Sickness on Plum Island for ten-years. African Horse Sickness virus can~be transmitted, by mosquitoes. As some people may know, Dr. Callis is a. horse lover. He gave up horseback riding because they.work on Plum Island oD a horse disease that does not occur in the United States. There are other diseases with which ~hey have worked which a~e transmitted by mosquitoes. Duck Plaque is a disease they haveworked with on Plum~Island sinee 1967; they made the d~agnosis of the problem in the d~ck industry right on Long Island. They worked on it quite safe~y for a number of years. During that time they samples many species of wildlife, birds that inhabit Plum Island. They~did this in collaboration with the Fish and wiTdlife Service and they found no antibodies in those species whatever. Dr.-Callis sincerely believes that the precautions which they have taken to work on Rift Valley Fever on Plum Island~are adequate to sustain thiS~virus in the facility in which~they~plan to wQrk on it~which is not going to be the whole Island, one laboratory~buitding.i They are going to take. a series of precautions in the laboratory building~ they will work on~ it only w~thin enclosed hoods and in the animal rooms they will-wear masks and will,be provided with an ~outside supply of air. They are~protecting the workers on ,Plum Island, those who are going to be e~p~s~ut there are in excess of 200 people they don't ~ think it is necessary to ~mmunize. All 3~0 of them live on the North Fork, they have families too, de we think-they would not be con- ~-~¢e~ke~~ their loved ones. Dr. Callis assurg~t~at he is just~as concerned about'h~s family as anyone else on the North Fork and he has no_intention of doing~anything ~%at-would endanger,their health or anyone elses life~ The other item Dr. Callis wished todiscuss was the incident on~ September 15th when they-diagnosed Foot a~d Mouth, Q~sease in their normal animals in their animal holding facility on Plum Island. This to them was a difficult thing~tQ face but they quickly made the diagnosis in a matter of two and a half hours and without 11o determining ju~st how many animals ~er~'-infected, they were al~ moved to the inside~ of the laboratory where--they ~n take-care'Of S~h: things .and/or were 'slaughtered% In the mea~time, they~ha~ve de~ c'ontamina~ted~.t~e facilities on the Island which,may have bee~'.~on~.~ taminated with th~ vir%s.- ~hey~are no~-l~oking-into j~st ~o~-the- virus escaped from the laboratory buiTding. Their investigat.i~n~is ~o~..~finished. They expected it to'be finished by- this Friday~b~t th~ ~re led to the~conclusion that i~ i% most likely in'some wa~ associated with the construction program which is under"way,'on plum Island at one laboratory building ore'some operating~procedure~hich was changed to accommodate the construction program. Obviously, if you are going to add on to s building you have to l~t the con- tractor get next to the building'and do the new.~onstruction and then add on to it. They,. fortunately, had plans for this eventuality and were able to implement those at a-mom~'nts notice and he feels quite certain that the~virus was contained w~in that one small group of. animals-on Plum Island and that the virus did not get off the'Island to th~ livestock on Long Island or Connecticut or Rhode Island, To make sure that that didn't happen, ~some of the livestock on the North Fork have been examined over the last two weeks just to make certain that they are free of any sign'of Foot,and .Mouth Disease.and the same precautions are being taken in Connect~R~ ~'~ Rhode Is.tand.~They have every belief they will b.e able'to/cause ~and must find the cause so they can get on with their mission. The regular meeting reconvened at~4:00~P.M. Mr. Nelson A~. ien posed several questions~of Dr. Callis. One was whether Rift~Valle¥ Fever is an RA or DA virus, giving' a lehgthy list of side effects, of RA and DA virus~and the effects of Rift' Valley Fever as published in several~different government and~medical publications. Dr. Callis stated it is RNA. Miss Hatfield, sitting with Mr. Axien also read several~ lengthy excerpts from various medical and gOvernmental journals. Dr. Callis stated that D~.~Walker was in Egypt and saw first ~and the effects of Rift Valley'Fever. I~ seems to affect the young of the sheep family, wiping: out the~ entire crop of young sheep while the adults live through it. ~Dr. Callis stated that in~'man, Rift Valley Fever causes a mile influenza like symptoms. In many countries of the world, there is large pop- ulations and Very l~ttle food, consequently, there are people of all ages who~are un~er~-nourished. They don't have enough-.pro~ein, ~ either plant ~or~ ahim~al, and mostly'they don't get much animal pro- tein, ~let alone p~ta~nt protein. TherefOre, a disease like R~ift Valley Fever, which normally-does not~ ca~Use'a high incidence of even adverse illness, ~u% complicated by the fact the populatiOn iB und~r-hourished th~n the disease incidence and the mortality is higher. That seems to' explain what is h~ppen~ng in,Egypt. Dr. Callis stated it ~s n~t a slow virus, it manifests symptoms within a matter of four or. f~ive days. Mr. Axien continued to read several symptoms of Rift Valley Fever,-as did Miss Hatfie!d. Mr. Axien spoke about the high inci~ dence of mosquitoes in~.recent years and the possibility of them carrY~iR~% Valley_Fever because we don't have~ th~-~s~al controls we had several years ago. ~iss Jody A~ams spoke about the-high incidence of,ticks,,and added that she:feels_a public hearing~shoutd be ~eld on the ma:t~er of Rift ~v, allev Eever, , NLr~--~enry Lytle-also stated there should be a separate hearing on the matter and the Town Board cannot do anything about~ it legally~ it is-something that is done from a federal level all the way down the line. Supervisor Martocchia.stated that the Board-felt they had enough interest in. Rift V~lle¥, Fever and the stir it has=caused in the community to invite'Dr. Callis over to:properly bring us up to date. Miss:Hatfield continued-to read from certain publications, at which time~,:Supervisor~Martocchia .stated he will try £o make arrangements with Dr. Callis and members of his staff for a meeting~at~ano~her date.~---The Board~-returned to the-regular agenda-of the meeting at 4~30 P~M.
