In connection with an application for a pre-existing Certificate of Occupancy
for a three-family dwelling and accessory detached garage on a parcel situate at Montauk
Avenue on Fishers Island, I have enclosed the following:
1. Completed Application for Certificate of Occupancy, signed by me.
2. Consent to Inspection, signed by the owner, Carolyn Juleen Doherty.
3. Survey of the premises prepared by CME Associates Engineering,
Land Surveying &Architecture, PLLC, dated December 21, 2011.
4. My check to the Town of Southold in the amount of the $100
application fee.
The Consent to Inspection form is a bit truncated due to the fact that the blank form was
e-mailed with another document and the format was disrupted in the process. I can have.
Mrs. Doherty sign another one if necessary.

Dear Gary:
In connection with an application for a pre-existing Certificate of Occupancy
for a three-family dwelling and accessory detached garage on a parcel situate at Montauk
Avenue on Fishers Island which you inspected last month, I have enclosed original letters
from the owner, Carolyn J. Doherty, and from the following former tenants: (i) Matt Reale,
(ii) William Bloethe, (iii) Tathiana Lema, (iv)'James Rogers (v) James Laine.
The enclosed letters attest to the fact that the property has been used as a
three-family dwelling, at least until the end of 2010. When you'inspected the property with
David Burnham,.you suggested that affidavits be obtained from former tenants. Although
the enclosed statements are not in affidavit form, I could prepare affidavits if necessary and
the persons who signed the letters have indicated in the last sentence that they are willing
to make the statements under oath if requested or required.
Please considerthe enclosed in making your determination as to whetherthe
Town will issue a pre-existing Certificate of Occupancy for a three-family dwelling as
originally requested and let me know if ,you require any further information or
documentation (including statements in affidavit form) before doing so.'

I, Carolyn Doherty, owner and lessor of the above named property, due hereby certify
that the afore mentioned property has always been operated as a three family dwelling.
This has been the case from well before I obtained ownership from the Gada Family
Partnership as well as prior to that,when owned by Gada General Contracting.
Tenants are required to pay their own telephone and electric charges. There are four
separate electric meters on the premises. Since there is only one furnace in the building,
the heat and hot water charges were included in the rents.
As a result of wishing to sell the property during the year of 2011, leases were terminated
and the building was closed at the end of December 2010.
