Woychuk Land Surveying, PLLC, last dated November 10, 2023, and stamped approved on November 15, 2023. The Permittee is required to provide evidence that a:copy of this Trustee permit has been recorded with the Suffolk County Clerk's Office as a notice covenant and deed restriction to the deed of the subject parcel.
