This permit will expire upon the date specified or upon a change of the Storage Owner. This permit is not transferable and is
granted subject to compliance with the provisions of Articles 12&18 of the Suffolk County Sanitary Code and 6 NYCRR Part
613. This permit can be revoked upon failure to follow the applicable provisions of these parts or comply with any special
conditions of issuance. All unauthorized or accidental releases of toxic or hazardous liquids must be reported to the Suffolk
County Department of Health Services at(631)854-2501 and to the NYSDEC SY,:aiill I llotlhne at 14300-457-7362.

SITE OPERATING PERMIT
STORAGE OF TOXIC AND HAZARDOUS MATERIALS
Please see the attached Permit to Operate for storage of toxic or hazardous materials.
This permit must be conspicuously posted at the site where the toxic/hazardous
materials are located. Failure to post the permit may result in delivery operator
refusing to make a delivery of toxic/hazardous materials, including fuel oil.
The Permit to Operate does not imply that all storage is in compliance with all
applicable codes. The owner and operator of the site are responsible for ensuring that
all storage is in compliance. Access to the site shall be granted to representatives of
the SCDHS for compliance inspections during normal business hours. Violations of the
Suffolk County Sanitary Code are punishable by fines of up to $2,000 per day, per
violation. Violations of the New York State Petroleum Bulk Storage Code are
punishable by fines of up to $37,500 per day, per violation.
