This application must be filled in by typewriter or ink and submitted to the Building Department with the following:
A. For new building or new use:
1. Final survey of property with accurate location of all buildings,property lines, streets, and unusual natural or
topographic features.
2. Final Approval from Health Dept, of water supply and sewerage-disposal(S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains less than 2/10 of 1%lead.
5. Commercial building,industrial building,multiple residences and similar buildings and installations, a certificate
of Code Compliance from architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
B. For existing buildings(prior to April 9, 1957)non-conforming uses,or buildings and"pre-existing"land uses:
1. Accurate survey of property showing all property lines, streets,building and unusual natural or topographic
features.
2. A properly completed application and consent to inspect signed by the applicant.If a Certificate of Occupancy is

The Board Members reviewed the contours on the updated survey
which was referred by you and compared with the November 1994
survey last filed with the ZBA Under Appeal No. 4295.
It,was deemed by the Board Members.at our Regular ra eeting heK,,on

marked: ,,fie_ point where the Ott ft. setback is shown for use when
phyab� W_'`measuring the setback at the site. The samo marker can
be used for other measurements as well, when needed.
In the future it would be best if the surveyor, when originally
applying for a variance, would show the contours at the highest
elevation and the reasoning for placing the "average top of the bluff
line" at a different location than the actual top of the bluff.
It is requested that a survey be filed with our office to show that
the swiiingpool has been properly placed from an In-ground marker.

P.S. Ken Abruzzo, surveyor with Young & Young has confirmed that the
101.7 ft. el. and "top of the bluff line determined by Young & Yaung on
Nov. 18, 1994" is the same line shown on the last map considered by the
ZBA under Appeal No. 4295. The ZBA Members have requested an in-ground
pipe or monument be planed at this 101.7 ft. el. as reference when
measuring setbacks. (The 101.7 el'., is shown on the March 1995 map on
file with the Building Department.) There was no objection to proceeding
based upon the above facts being true and aecurate.
