of Code Compliance from architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.

features.

We are contemplating some renovation work to the detached accessory garage at the
above referenced property and would like to have a Pre-CO inspection. The garage was
built in the early 1900's and has not been touched since the 1930's. Enclosed is a CO
application; signed Consent to Inspect form; copy of a recent survey; and a check for$100.
Please feel free to contact me with questions or for any-additional information.
Please note that it is just the garage to be inspected, not the main house. Thanks very

Sharon (Rafferty) Patterson, who is a resident of Suffolk County, State of New York, and
makes this her statement and Affidavit upon oath and affirmation of belief and personal
knowledge that the following matters, facts and things set forth are true and correct to the
best of her knowledge:
My name is Sharon Patterson, nee Rafferty, a full-time resident of Fishers Island,
NY. I was born in 1942; 1 am 76 years old. In 1954, my father purchased the house
currently owned by the 375 East End Road Realty Trust, 33 Treasure Pond Road,
Fishers Island, NY. I spent my childhood in the house. I attest to the fact—from
