This application must be filled in by typewriter or ink and submitted to the Building Department with the following:
A, For new building or new use: 1. Final survey of propm'ty with accurate location of all build/ngs, property lines, streets, and unusual natural or
topographic features
2. Final Approval fi:om Health Dept. of water supply and sew~age-disposal (S-9 form).
3. Approval of electrical installation from Board 0f Fire Underwriters.
Sworn statement from plumber certifying that the sold~r used in system coma/ns less than 2/10 of 1% lead.
5. Cmxm~ercial building, industrial building, multiple residences and similar buildings and installations, a certificate
of Code Comphance f~om architect or engineer responsible for the building,
6. Submit Plauning Board Approval of completed site plan requirements.

Dear Mr. Fish:
Pursuant to our conversation of yesterday, and my field observations
kindly receive this letter as my full acceptance of the construction
of the above mentioned project.
In specific:
- All framing as constructed.
- Distance in crawl space from bottom of joists to the existing
~concrete slab to be 12" in lieu of 18".
-Concrete foundation/footing is 9" wide, trenched out to 14" at the
bottom, with 2 courses concrete block at top, filled solid.
- There will be a 4' spiral stair installed in lieu of the stairs as
shown on t~. plans, due to space restraints in the cellar.

OF THE OWNERS OF THE PREMISES KNOWN AS #190 ANGLER'S
ROAD GREENPORT, NEW YORK, HAVE BEEN ASKED TO ADDRESS
SOME CONCERNS THAT AROSE DURING A RECENT INSPECTION
MADE BY THE TOWN OF SOUTHOLD.
WITH REGARD TO THE ABOVE MENTIONED RESIDENCE, THERE ARE A
NUMBER OF CRACKS, AND TORN TAPE JOINTS 1N CORNERS AND AT THE
CEILING, WALL JUNCTIONS. THIS IS THE RESULT OF SOME INITIAL SET'FLING, AND
FRAMING SHRINKAGE POSSIBLE DUE TO A HIGH MOISTURE CONTENT AT THE
TIME OF FRAMING. THIS CONDITION tN NO WAY DIMINISHES THE ORIGINAL
STRUCTURAL STABILITY, AND OR INTEGRITY OF THE HOUSE. IN ADDITION I AM
CERTIFYING THAT THE REAR DECK RESTS ON WOOD POSTS ON CONCRETE
PIERS, AND IS DESIGNED TO MEET A REQUIRED 40# PER SQUARE FOOT DESIGN
LOAD. THANK YOU FOR YOUR UNDERSTANDING IN THIS MATTER.
