for the property identified above, the New York State Department of
Environmental Conservation has determined that:
The parcel is more than 100 feet from reg~hlated, freshwater
wetlands. Therefore, no permit is requ/redtznder the Freshwater
Wetlands Act (Article 24 of the Enviror~ental Conservation Law).

Dear Mr. Vedty:
We have performed surveying services 0ursuant to the request of Mr. & Mrs. Smith. building oermit
applicants with vacant land on Soundview Ave. in Mattituck. The Smith s asked us to obtain set back
distances of their neighbors within 300' of their property on Soundview Ave.
Our findings in the field are as follows: in a southwesterly direction toward Dogwooc Lane. there
are two dwellings within 300' of the subject property along Soundview Ave. The adjacent nome under
construction has a set back from the road of 40.2'. The home of Marcelo Sehons on the comer of
Dogwood Lane and Soundview Ave. has a set back of +/- 15'. In a northeasterly direction from the
subject property there is one dwelling within 300' along Soundview Ave., the home of Vincent Carnag~e
which has a set back of 29'.
The average set back of these three dwellings is approximately 28'; please advise as to the impact of
our findings an Mr. & Mrs. Smith's proposed house set back.

General Notes:
1. Floors:
All floors over outside air or unconditioned spaces to have R-19 insulation.
2. Air Leakage:
Joints, penetrations and all other such openings in the building envelope that are
sources of ak leakage shall be sealed.
Recessed lights must be Type IC rated and installed with no peneirations, or Type
IC or non - IC mmd installed inside an appropriate ak-tight assembly with a 0.5"
clearance from combustible materials and 3" clearance fi.om insulation.
3. Vapor Retat'der:
Required on the warm-in-winter side of all non-vented framed ceilings, walls, and
floors.
4. MatelqalS identification:
M~tterials~ and equipment must be installed in accordance with the manufacturer's

6. Duct Construction:
All joints, seams, and connections must be securely fastened with welds, gaskets,
mastics (adhesives), mastic-plus-embedded-fabric, or tapes. Duct tape is not
permitted. Exception: Continuously welded and locking-type longitudinal joints
and seams on ducts operating at less 2 in w.g. (500Pa).
Ducts shall be supported eve~/I0 feet or in accordance with the manufacturer's
instructions.
Cooling ducts with exterior insulation must be covered with a vapor retarder.
Air filters are required in the return air system.
The HVAC system must provide a means for balancing ak and water system.
7. Temperature Controls:
Each dwelling unit has at least one thermostat capable of automatically adjusting
the space temperature set point of the targes~ zone.
8. Fireplaces:
Fireplaces must be installed with fight fitting non-combustible fireplace doors.
Fireplaces must be provided with a source of combustion air, as required by the
Fireplace construction provisions of the Res~en~ial Code of lVew I~ork Stat~
Service Water lteating:
Water heaters with vertical pipe risers must have a heat trap on both the inlet and
outlet unless the water heater has an integral heat trap or is part of a circulating
system.
