This application must be filled in by typewriter OR ink and submitted to the building
inspector with the following: for new building or new use:
i. Final survey of property with accurate location of all buildings, property lines,
streets, and unusual natural o¥ topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal(S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains
less than 2/10 of 1% lead.
5. Commercial building, industrial building, multiple residences and similar buildings

the courts of the State of New York, hereby affirms upon infox,nation and

I am the attorney for the Thomas Tsoanis, and I make this Affirm fion
in support of a certificate of occupancy of pre-existing use application.
Thomas Tsounis owns property located at 955 Inlet Drive, Mattituck,
New York and further described as SCTM No. 1000-099.00-02.00-014.002.
Mr. Tsounis purchased a share of this property from his parents in
1985, and then full ownership in the year 2002.
This property was a part of a subdivision designated as lot 76 as
shown on a certain map entitled, "Map of Captain Kidd Estates" filed in the
Suffolk County Clerk's Office on January 19, 1949. A copy of the
description of the property is attached hereto and made a part hereof as
Exhibit A.
Our tifie report evidences that a developer named Dawn Estates, Inc.
sold the premises to John Hoch and Helene A. Hoch on March 12, 1955,
which is the time that the construction of the house must have been
completed. A copy of the page of the title report is attached hereto and
made a part hereof as Exhibit B.
WHEREFORE, the circumstances make it clear that the house was
built prior to the year 1955.

BEGINNING at a point on the northerly side of Summit Drive at the extreme westerly end of an arc of a curve which
connects the northerly side of Summit Drive with the westerly side of Inlet Drive;
RUNNING THENCE South 83 degrees 40 minutes 00 seconds West along the northerly side of Summit Drive 95 feet to a
point;
THENCE North 16 degrees 15 minutes 50 seconds West 101.86 feet to a point;
THENCE North 85 degrees 21 minutes 40 seconds East 137.62 feet to the westerly side of Inlet Drive;
