Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains less than 2/10 of 1% lead.
5. Commercial building, industrial building, multiple residences and similar buildings and installations, a certificate
of Code Compliance from architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.

Dear Mr. Raynor
Thank you for the cooperation during the fire inspection of Matt-A-Marr Marina on April
9, 2003.
As of the above date, all violations from the previous fire inspections have been
corrected.
Please be advised Building Permits are required for all other structures without proper
permits and certificates of occupancy. This procedure should be started as soon as
possible.

Fire Inspection Report 2003

Dear Dr. Agarabi:
Thank you for the cooperation during the fire inspection of Matt-A-Marr Marina on
January 28, 2003.
During the inspection the following violations were noted and should be corrected
immediately.
Office:
Exposed insulation to exhibit a flame spread index of not more than 25 and a
smoke-developed index of not more than 450. A 15-minute fire barrier would
suffice.*
Fire extinguishers shall be inspected monthly by a designated employee and
inspected and tagged yearly by an approved servicing company. *
Hand held portable fire extinguishers, not housed in cabinets, shall be installed
on hangers or brackets supplied. Hangers or brackets shall be securely
anchored to the mounting surface according to manufacturer's specifications. *
Open junction boxes, open breaker spaces, and open wiring splices shall be
prohibited. Approved covers shall be provided for all switch and electrical outlet
boxes. *
Compressed gas containers, cylinders and tanks shall be secured to prevent
falling caused by contact, vibration or seismic activity. *

The electric service and wiring in the building shall be certified proper and
acceptable by an inspecting agency or an engineer. *
Store:

Meter
Boxs: (Center and South)
Open junction boxes, open breaker spaces, and open wiring splices shall be
prohibited. Approved covers shall be provided for all switch and electrical outlet
boxes. *
Restaurant:
The electric service and wiring in the building shall be certified proper and
acceptable by an inspecting agency or an engineer. *
Extension cords and flexible cords shall not be a substitute for permanent wiring.
Extension cords and flexible cords shall not be affixed to structures, extended
through walls, ceilings or floors, or under doors or floor coverings, nor shall such
cords be subject to environmental damage or physical impact. Extension cords
shall be used only with portable appliances. *
Exits shall be unobstructed at all times. The side exit has a curtain in the
doorway and materials stored in the exit path. These shall be removed. *
The ceiling in the kitchen shall be properly installed and maintained. *
The damaged stucco finish shall be properly replaced and repaired.
Kayak office:
Fire extinguishers shall be inspected monthly by a designated employee and
inspected and tagged yearly by an approved servicing company. *
The electric service and wiring in the building shall be certified proper and
acceptable by an inspecting agency or an engineer. *
A working space of not less than 30 inches wide, 36 inches deep, and 78 inches
high shall be provided in front of electrical service equipment. No storage of any
material shall be stored within the designated working space. *
An approved engineer shall certify that the flue pipe from the heater and its
installation is proper and acceptable. *

Propane tanks in the basement shall be removed. *
Rubbish and unused materials shall be removed from the basement area. *
Storage Buildinq:
Fire extinguishers shall be inspected monthly by a designated employee and
inspected and tagged yearly by an approved servicing company. *
Combustible waste, refuse, and large quantities of dry vegetation, which by
reason of their proximity to buildings or structures would constitute a fire hazard
or contribute to the spread of fire, shall be removed. *
Bathroom Cleaning Room:
The floor is not proper· An engineer shall certify the floor proper and acceptable·
Pool House:
The flammable liquids shall be stored in proper remote location. *
The building shall be properly placarded indicating the storage of hazardous
materials inside. *
The items indicated by an (*) indicate a fire and/or a safety hazard.

The boat rack system currently in place at Matt-A-Mar Marina on the Sound is
portable and is being used solely for winter storage of boats. At the close of
the winter storage season, approximately May 2004, the rack system will be
removed.
